Actions aimed to improve mental health primary care in Florianópolis, SC, Brazil

O presente estudo busca realizar um diagnóstico da atenção à saúde mental no contexto da Atenção Primária no município de Florianópolis. Para tanto, utilizamos como instrumento para a coleta de dados entrevistas reali¬zadas com coordenadores de unidades locais de saúde. Os resultados aponta¬ram para o recente surgimento de um trabalho mais específico em saúde mental, embora os relatos pareçam indicar que as equipes ainda desenvolvam suas ações direcionadas mais para a realização diagnóstica e reabilitação do que para a promoção em saúde e prevenção de agravos maiores decorridos do sofrimento psíquico. Este estudo procurou contribuir para o conhecimento do cuidado realizado em saúde mental na atenção básica, trazendo elementos para discussões pautadas na realidade do município e na perspectiva de colaborar para o aprimoramento deste serviço.

This study aims to do a diagnosis of mental health care within the context of Primary Care in Florianópolis, SC. Thus, interviews were conducted with coordinators of local health units to collect data besides documental analyses. The results showed that a more specific approach has been recently adopted in mental health, even though the comments of those interviewed signal that the working teams actions are still focused on diagnosis production and rehabilitation rather than on health promotion and prevention of further damage resulting from the mental illnesses. This study attempted to contribute to a deep understanding of mental health care provided by Primary Care Units, by offering and opportunity to discuss this issue based on the local reality, and collaborating for the improvement of this service.

Science children: assisted reproductive motherhood

Este artigo propõe a diferença conceitual entre ser mãe, segundo a Biologia e ser mãe, para a Psicanálise. A experiência clínica com mulheres ditas inférteis, no Centro de Reprodução Assistida do ICHC-FMUSP, indica que a demanda pela gestação pode não corresponder, necessariamente, à demanda de ser mãe. Dessa experiência conclui-se que ser mãe é ocupar um lugar psíquico, ainda que a gestação possa ser crucial para a construção da função materna; que a impossibilidade de ser mãe pode estar relacionada com a dificuldade de ter filhos; e que a atenção a elementos inconscientes diminui o risco do nascimento dos filhos da Ciência.

This article proposes to analyze the conceptual difference between being a mother, according to Biology and Psychoanalysis. The clinical experience with infertile women, at the ICHC-FMUSP-Assisted Reproduction Center indicates that the demand for pregnancy may not necessarily correspond to that of being a mother. It leads to the conclusion that being a mother is to fill a psychic place, despite the fact that pregnancy might be crucial for the construction of the nurturing function; that the impossibility of being a mother may be related to the difficulty of having children; and that attention to unconscious elements decreases the risk of producing Science children.

Education and psychoanalysis: educative practice and the production of subjectivity

Podemos encontrar no campo educativo o predomínio hegemônico da tese de que toda relação de ensino e aprendizagem deva materializar-se em ações que resultem em efetivas trocas de práticas entre os sujeitos, e que isso possa também resultar na constituição ou na alteração de determi-nados tipos de comportamento do educando. Essa ideia apoia-se na hipótese de que o sujeito educado é aquele que desenvolve práticas observáveis e passíveis de avaliação em função das expectativas do sujeito educador. Neste artigo, o ponto central da análise é que educamos na crença de reproduzimos a nós mesmos, ou seja, educamos para formar o cidadão educado à nossa imagem e semelhança. Entretanto, o outro pode constituir-se na diferença. A educação, portanto, pode resultar em práticas que reprodu-zam a subjetividade ao realizar o eu no outro ou na produção do inédito.

We can find in the educational field the hegemonic predominance of the thesis that all relation between education and learning must materialize into actions of effective exchange of experiences between individuals, also resulting in changes of certain types of behavior of the individual being educated. This idea is supported by the hypothesis that the educated citizen is one that develops observable practices that can be evaluated in function of the expectations of the educator. This article is focused on the belief that we educate to reproduce ourselves, that is, we educate others based on our own image and similarity. However, the other can be constituted by difference. The educational process therefore, can result in practices that reproduce subjectivity by constructing I in the other or in producing a new I.

The progressive discharge as a social reintegration means of the judiciary asylum patient

A alta progressiva (AP) é um benefício concedido pela justiça a pacientes que estão cumprindo medida de segurança em manicômio judiciário. Trata-se de uma prática de desinternação que visa a reinserção social dos indivíduos considerados inimputáveis e que cometeram delitos. Este artigo tem como objetivo apresentar o histórico e os principais conceitos norteadores da AP, além de apontar as características desta prática, seus procedimentos e suas etapas. Serão abordados aspectos teóricos sobre o surgimento da AP, sua caracterização e sua relação com a Reforma Psiquiátrica. Essa desinternação gradual pode ser considerada como ferramenta terapêutica para o tratamento e para a avaliação dos pacientes, um meio de controle de reincidência delituosa, além de servir de modelo para os hospitais de custódia do País.

The progressive discharge (PD) is a benefit granted by the Judiciary to patients who are complying with security measure in a judiciary asylum. This is a discharge practice that aims the social reintegration of the individuals considered irresponsible and who committed delicts. This article aims to present the history and the main concepts guiding the PD, besides outlining the characteristics of this practice, its procedures and stages. It will be addressed theoretical aspects on the appearance of the PD, its characterization and its relationship to the psychiatric reform. This gradual discharge can be considered as a therapeutic tool for the treatment and evaluation of the patients, means of criminal recidivism control, and it serves as a model for the custody hospitals of the country.

Work and madness: career possibilities for the psychotics

Este artigo levanta os sentidos construídos sobre trabalho e carreira de um grupo tradicionalmente excluído do trabalho, ou seja, as pessoas em situação psicótica, e suas estratégias de construção da trajetória no mundo de trabalho consideradas de difícil concretização. Foram formados grupos focais com 30 pessoas com transtornos mentais psicóticos. Os resultados indicaram que a maioria dos sujeitos associa trabalho com saúde e vê a carreira impossibilitada pela situação psicótica, entretanto uma parte deles está trabalhando e diz se sentir capaz de construir uma carreira pela maior receptividade do atual mercado. Conclui-se que o estigma e a impossibilidade de carreira ainda são predominantes, mas que a atual conjuntura de transição mundial parece estar criando estruturas psicossociais mais receptivas ao diferente.

This article analyzes the meanings of work and career for a group traditionally excluded from work (the psychotic) and their strategies for career development difficult to implement. Focus groups were conducted with 30 people with psychotic mental disorders. The main findings pointed out that most participants make a connection between work and health and believe that it is impossible for them to hold a career because of their psychosis; however, some of them work and feel capable of building a career encouraged by the new market diversity. It was concluded that the stigma and difficulty in developing a career still predominate, but the transitional aspect of the world seems to be creating new psychosocial structures more open for the differences.

Care starts with listening: mental health professionals and practices vicissitudes

Cabe aos profissionais de saúde mental, em grande medida, efetivar diretrizes da reforma psiquiátrica em suas práticas. Isso exige adequações desde a formação em saúde até o desenvolvimento em serviço. Este trabalho objetiva investigar concepções sobre especificidades do trabalho em saúde mental, como também relatar a experiência de espaços de cuidado com profissionais de três equipes de saúde mental, buscando contribuir para estratégias de educação permanente. Foram realizadas entrevistas semiestruturadas individuais com 48 profissionais. As entrevistas foram gravadas, transcritas e analisadas qualitativamente. Os resultados apontam sobrecarga emocional; necessidade de afinidade com a área; necessidade de capacitação; necessidade de redução da carga horária; necessidade de melhora na comunicação e funcionamento da equipe; e necessidade de espaços de cuidado para os cuidadores.

It is the health professionals responsibility, to large extent, to make psychiatric reform goals effective in their practices. Adaptations are necessary throughout their health formation up to service development. Thus, this work aims to investigate the conceptions health professionals hold of the specificities of their work, and to report the care opportunity experiences with professionals from three mental health staffs, searching to contribute for strategies of permanent education. Individual semi-structured interviews of 48 professionals were recorded, transcribed and qualitatively analyzed. The results obtained showed emotional overload as well as the need to improve affinity with the mental health area, to increase the number of qualified personnel, to reduce workload and to improve team communication and functioning, as well as to create spaces where professionals could be emotionally cared.

A case that talks to the net: the analytical listening in the articulation of menthal healt care

O artigo objetiva discutir a especificidade da atuação do psicólogo nas equipes de assistência em saúde mental, em especial o lugar reservado à escuta analítica nesse contexto. Para consecução deste objetivo, será dado relevo ao recorte clínico do caso de uma adolescente acompanhada no Estado do Rio de Janeiro pelas mais diferentes equipes do campo da Saúde Mental, da Assistência Social e até mesmo da Justiça. Esperamos questionar a inserção do psicanalista em tais equipes e a responsabilidade implicada em sua escuta na articulação com as demais propostas de acompanhamento diante os impasses colocados pela problemática da adolescente em questão.

This article discusses the specificity of the role played by psychologists as members of Mental Health care teams, and, in particular, the place reserved for analytical listening within this context. To reach this objective, emphasis is given to a fragment of a clinical case of an adolescent followed up by different Mental Health and Social Assistance teams and even by the Justice, in the state of Rio de Janeiro. Our aim was to question the participation of a psychoanalyst in such teams and his responsibility in listening, combined with the other follow-up proposals in face of the difficulties presented by the case under study.

Academic writing and self writing: experiences challenges

Este ensaio problematiza os modos tradicionais de escrita acadêmica inspirado nas experiências de alguns intercessores que potencializaram escritas desubjetivantes a partir da interface pensamentos e afetos. Refletimos sobre o modo de operar nossos escritos acadêmicos de forma a exercitar uma escrita de si, uma escrita-potência. A ideia central é que na construção de nossos trabalhos acadêmicos podemos experimentar a nós mesmos por diferentes modos de ser, no ato em que produzimos tais textos. Essa possibi¬lidade é uma ação definida por Foucault e demais intercessores do campo da arte e da filosofia da diferença como exercício de liberdade, pois trata, efetivamente, de investirmos um movimento sobre nós mesmos, em que estilizamos, no ato em que compomos nossa escrita, nossa própria existência.

This essay focus on the traditional modes of academic writing based on the experiences of some authors who, when producing potentially subjectivating texts, are guided by their own thoughts and feelings. We make a reflection on the ways we can produce academic writing so as to create empowered self-writing. The main idea is that we can experiment ourselves through different ways of being when producing academic texts. This possibility is an action defined by Foucault and others in the field of art and philosophy of the difference as an exercise in freedom, since it is, effectively, a matter of looking inside ourselves, recreating our own existence through our writings.
