Psychosis treatment: the social string as an alternative to the ideal institution

A partir de discussões realizadas na Sessão Clínica do Instituto Raul Soares, foi possível refletir sobre os impasses do tratamento da psicose em uma instituição de saúde mental que tem como diretriz de trabalho a reforma psiquiátrica. Se os ideais preconizados pela reforma forem considerados universalizantes e desconsiderar a particularidade de cada sujeito, eles poderão ser, muitas vezes, indutores de um surto e não propiciadores de uma estabilização em sujeitos psicóticos. Ao tomar como referência o caso Márcia, demonstra-se como as intervenções baseadas no ideal institucional e não na construção do sujeito podem desencadear uma desestabilização. Deste modo, é necessário precisar a forma de conduzir o tratamento, sem ter como premissa os ideais de inserção social (autonomia), mas o estabelecimento de um possível laço social na psicose.

ABSTRACT After some discussions at Raul Soares Clinic Session, it was possible to reflect over the impasses of the psychosis treatment in a mental Health institution, which has as an objective a "Psychiatry Reform". If the If the ideal of social insertion and the autonomy preconceived by the reform were considered universalized, disregarding any kind of particularity, they may be, many times, inductors of a breakdown and not a reaction of stabilization in psychotics patients. Considering the case Márcia, can be demonstrated as interventions based in the institutional ideal and not in the construction of a subjectivy to permit an instability, as the agitation psychomotor and hallucinations. By this means, it is necessary to conduce the treatment precisely, disregarding the social insertion; autonomy; but, considering a social string in psychosis.

Clinic session: about the effects of institutional intervention

Este artigo terá como objeto, a Sessão Clínica do IRS, com ênfase na repercussão institucional desde trabalho - a discussão do caso Marlene, um exemplo paradigmático de interpelação à instituição, que coloca o questionamento: o que fazer com um sujeito em crise, em um quadro complexo o bastante para impossibilitar sua liberação, mas sem gravidade suficiente para justificar uma internação? Tomar essa questão, não pela vertente da discussão teórica, mas sob a perspectiva da construção de um caso clínico, levou a equipe a interrogar a lógica de funcionamento do setor e introduzir uma mudança de perspectiva a respeito da função da urgência, assim como da articulação da equipe. Mostraremos as questões que foram suscitadas neste primeiro encontro, e o efeito de mudança que pôde ser constato, ao longo do ano, em outros casos levados para a Sessão Clínica pela equipe da urgência.

ABSTRACT This article aims the IRS Clinic Session, emphasizing the institutional repercussion of this work. The discussion about Marlene Case brought us a question: what we should do with a patient in crises, in a complex situation where could not be discharged, without major seriousness which wouldn't justify an admission at the hospital? Considering this question, not by the theoretic discussion, but for the construction of a clinic case, it led the group to question the sector logical function, introducing a change in the perspective about the urgency factor, as well as the group procedure. We will show the questions which were brought in this first meeting, and the effect of change that was noticed, through the year, in other cases brought to the clinic session, by the urgency group.

Discussion x clinical case construction

Acerca de quatro anos é realizada, no Instituto Raul Soares, a Sessão Clínica - um espaço destinado à discussão dos casos clínicos. Esse espaço para discussões semanais é uma estratégia de intervenção na instituição e de construção dos casos clínicos. Os casos considerados mais difíceis, que apontam à instituição o seu limite de resposta, vão à sessão clínica, que tem como método de trabalho a entrevista com o paciente e ou a apresentação do caso pela equipe. Inspirada no trabalho de Lacan, a Sessão Clínica traz a marca da articulação entre psicanálise e instituição: a psicanálise é aplicada considerando a dimensão singular de cada caso, o que implica no favorecimento de sua construção e na atuação da equipe de acordo com a lógica da prática feita por muitos. Neste artigo será trabalhada, a partir desta experiência, a noção de discussão em contraposição à construção do caso clínico.

ABSTRACT For four years, is has being executed at Raul Soares, the IRS Clinic Session: There is a place designed to discuss the clinical cases. This place of weekly discussions, is an strategy of intervention in the institution an in the Construction of the Clinic Case. The case, which are considered the most difficult and which requires from the institution an answer, attend the Clinic session, which has as approach an interview with the patients as well as a presentation of the case by the group responsible for the patient. Inspired by the work of Lacan, the clinical session brings the mark of the institution psychoanalysis articulation: Psychoanalysis, applied bringing a singular dimension of each case, trying to make it favorable to construct each case and making it possible for the group to act accordingly to the practical logic done by many. In this article will be worked, from this experience, the notion of the discussion in counter- balance to the construction of the clinical case

The clinic session as the articulation of the knowledge diversity

Em uma equipe multidisciplinar tem-se uma diversidade de discursos e teorias que, muitas vezes, por se pautarem em diferentes concepções da doença, imprimem diferentes rumos para um tratamento, e acabam por produzir efeitos clínicos desfavoráveis, desde a não resposta ao tratamento, desestabilização, reagudização do quadro e, até mesmo, a passagens ao ato. A sessão Clínica do IRS, ao tomar a construção do caso clínico como método de investigação e intervenção, funciona como um articulador dessa diversidade de saberes, ao reunir elementos dos diferentes discursos para encontrar a posição singular do sujeito em relação ao gozo. Neste artigo, essa função de articulação será demonstrada a partir do relato da construção de um caso, ao apresentar seus impasses, divergências, e as elaborações que possibilitaram redimensionar o tratamento.

ABSTRACT In a group of multi-disciplines there is a variety of speeches ans theories that, many times, act in diferent concepts of the disease, leading to diferent ways of a treatment, producing clinical effects unfavorable, since a non answer from the treatmen, an instability, an acute reaction in the process. The IRS Clinic Session, considering the Clinic Case Construction as a method of investigation and intervention, it work as an articulator within this diversity of knowledge, gathering elements of different discourses, to meet te singular position of the subject concerning the satisfaction. In this article, this articulation function will be shown by the report of the construction of a case, presenting its impasses, divergences, and the elaboration which permitted the redirection of the treatment.

The evaluation and its destinations

Com esse artigo contrapõe-se historicamente a avaliação praticada pela psiquiatria em sua relutância em considerar as terapias hermenêuticas. Com o advento de maio de 68 ocorrem mudanças na ideologia institucional, fato que permite a entrada dos psicólogos no tratamento do sofrimento mental. Atualmente, a avaliação na Itália é marcada pelo deslocamento do sintoma particular para o sintoma social (epidemia). Avaliação da implantação do Centro de Atenção Psico-Social em Cataguases - MG

Em decorrência da nova abordagem de tratamento dos portadores de transtorno mental, que prioriza o atendimento fora dos limites dos manicômios, começaram a ser implementados no Brasil os Centros de Atenção Psico-Social (CAPS), locais onde se procura recolocar o sujeito na sociedade, por meio de uma ação interdisciplinar, em que diversos saberes se colocam à disposição dele para reintegrá-lo ao mundo, com seus deveres e direitos. Com este trabalho tem-se como objetivos principais traçar o perfil dos usuários do CAPS de Cataguases, avaliar o funcionamento do serviço por meio de dados colhidos na clínica psiquiátrica de referência do município, avaliar o nível de funcionamento dos técnicos que trabalham na instituição e saber o impacto causado na sociedade pelas atividades desenvolvidas no CAPS. Foi realizada uma análise retrospectiva do histórico dos usuários do CAPS, entre setembro de 2002 a junho de 2004, período de funcionamento do serviço. Por meio dele foram atendidos 235 pessoas, enquanto o total de internações de pacientes de Cataguases em clínicas psiquiátricas, no mesmo período, foi de 276. A maioria dos usuários do CAPS é do sexo masculino, solteira, com idade entre 31 a 40 anos, mora com os pais, não possui dependentes, é aposentada e recebe um salário mínimo mensal. O diagnóstico de esquizofrenia paranóide foi o mais freqüente, com 48% dos usuários, seguido por retardo mental moderado, com 15%. Cerca de 25% dos usuários apresentaram comorbidades, sendo a epilepsia a mais freqüente (15%). A maioria dos usuários nunca foi internada em manicômio (45%), mas cerca de 5% já o foram, por mais de 10 vezes; destes, a totalidade vive em ambiente hostil, com baixo nível de qualidade de vida. A observação dos dados colhidos mostra que algumas ações devem ser prioritárias no apoio sócio-familiar dos usuários e que medidas administrativas devem ser tomadas na condução do processo de gestão do CAPS. A grande utilidade do trabalho está em fornecer dados para serem comparados aos de outros locais, já que a escassez de informações semelhantes faz com que muitas equipes ajam sem conhecer seu trabalho.

ABSTRACT The CAPS were implemented in Brazil in result of the new kind of treatment to the mental patient that prioritizes the attendance out of the mad-houses. In the CAPS, there is an effort to introduce the subject in the society, through an interdisciplinar action that tries to reintegrate him in the world, with his duties and rights. This work presents as main objectives to trace the profile of the Cataguases CAPS´s users, to evaluate the functioning of the service through data collected in the psychiatric clinic of reference of the city, to evaluate the level of functioning of the technician that work in the institution and to know the impact caused in the society for the activities developed in the CAPS. The majority of the users of the CAPS is men, single, adults (between 31 and 40 years old), live with their parents, don't have dependents, is retired and receives a minimum wage monthly. The diagnosis of paranoid schizophrenia was the most frequent (48% of the users), followed by oligophrenia (15%). About 25% of the users had presented problems like epilepsy (15%). The majority of the users had never been interned in mad-houses (45%), but 5% of them had already been interned for more than 10 times (these patients live in hostile environment, in a low level of quality of life). This research shows that some actions should be taken in order to reduce the problems of the CAPS´s patients and it also shows that some administrative actions must be taken in the CAPS´s management. The great utility of this article is to supply datas to be compared to the ones of the other places because there is little information available.

Three questions about the psychoses: a reading of the Anti-Édipo

Com o artigo problematiza-se três questões colocadas pela psicose: a da psicose como doença que interpela as normas sociais, a da perda da realidade no delírio e a do deslizamento da linguagem no esquizofrênico, a partir da leitura de O Anti-Édipo.

Clarifying the meaning of the mental deficiency: a psychoanalytical interpretation

RESUMO Com o presente artigo pretende-se desvelar o sentido da deficiência mental, segundo a abordagem da teoria psicanalítica. Discutir o sentido da deficiência mental representa um grande desafio para os profissionais que atuam diretamente com seus portadores. Contudo, não pretendemos fazer da deficiência mental ou dos deficientes mentais, entidades isoladas, mas colocá-los no movimento do sujeito em "devir", de um sujeito dividido pelo inconsciente.

ABSTRACT The present article intends to go further in the meaning of mental ilness acording to the psychoanalytic theory approach. Discuss the sense of mental illness represents a challenge to the professionals who deal directly with the people affected by it. However, it´s not our intention to consider the mental illness or the mental ill as na isolated entity but include them in the person´s "willcome" movement, a self divided by the unconscious.

The psychoanalyst formation and the eternal dream of his regulation

Nesse artigo discute-se a formação do analista a partir do conceito de real para a ciência e para a Psicanálise, e das categorias do necessário e do contingente. Com o texto, tenta-se demonstrar que o singular pretendido pela psicanálise estabelece a impossibilidade de uma regulamentação baseada em um saber de referência universal.

ABSTRACT The article discusses the analyst formation from the point of view of the scientific concept of real, the logical categories of the necessary and the contingent, and their specific use in psychoanalysis. The text argues that the particular sense intended by psychoanalysis imposes the impossibility of any regulation based in a universal referent knowledge.

The neoliberal capitalism and its subject

Utiliza-se o referencial teórico da Psicanálise para se empreender uma reflexão sobre a alienação do sujeito no capitalismo neoliberal de nossos dias. O percurso se inicia por meio da explicitação das concepções psicanalíticas de sujeito e sociedade, destacando-se a importância da castração simbólica na instauração do inconsciente e na inauguração do laço social. Registra-se a presença de uma violência originária que é componente estrutural desse processo, à qual se agregam modos adicionais específicos de violência do capitalismo neoliberal. Busca-se caracterizar dois aspectos especialmente acentuados nos tempos de intensificação contemporânea do neoliberalismo: o 'narcisismo' e a 'perversão' do sujeito, ainda que se deva colocar aspas nos dois termos, na medida em que se trata de noções distintas da concepção usual metapsicológica de narcisismo e do conceito de perversão como estrutura clínica do sujeito. Finalmente, destaca-se a existência de uma imensa produção cultural ideológica na mídia, nas ciências, na filosofia e nas artes, encarregada de oferecer sustentação ao status quo social e de pôr obstáculos ao progresso histórico e à transformação da sociedade.

ABSTRACT The psychoanalysis theory is used to ponder over the subjetc´s alienation in the neoliberal´s capitalism of our days. This paper begins with the exhibition of the subject and society´s psychoanalytical concepts and emphasizes the castration symbolic´s importance in the instauration of the unconscious and in the inauguration of the social network. In this article is mentioned the presence of an originary violence that is part of this process, to which is added new violence´s ways that come from the neoliberal capitalism´s system. The subjet´s "narcissism" and 'perversion' (two strong aspects of this neoliberalism´s era) are characterized differently from their usual ways. Finally, it is standed out the existence of an immense ideological cultural production in the media, in the sciences, in the philosophy and in the arts, that keeps the social´s status quo and sets obstacles to the historical progress and the transformation of the society.
