Therapeutic change in family therapy: constructing agency

Este artigo visa compreender o processo de produção de sentidos sobre a mudança em terapia familiar sob a perspectiva construcionista social. A análise do processo terapêutico de uma família apontou que a construção conversacional da mudança terapêutica foi marcada pelo deslocamento do discurso inicial do problema de saúde da mãe, oriundo do trabalho, para o discurso do agenciamento com o cuidado da vida. Tal construção foi analisada em relação a três aspectos centrais: (a) a desconstrução do sentido do trabalho; (b) a redefinição da relação mãe-filha; (c) a reconstrução da vida. A análise permitiu ainda compreender um importante processo subjacente, o agenciamento como forma de promover a mudança terapêutica em terapia familiar. Concluímos que a postura aberta, ativa e engajada da terapeuta com o fluxo da conversa favoreceu a emergência desses sentidos de mudança.

This article aims to understand the meanings making process about change in family therapy under the constructionist perspective. The analysis of the therapeutic process of a family pointed out that the conversational construction of the therapeutic change was marked by shifting the discourse from the initial health problem of the mother which arose from her work to a discourse of managing care in life. Such construction was analyzed in relation to three key aspects: (a) the deconstruction of the meaning of work, (b) the redefinition of the mother-daughter relationship, and (c) reconstruction of life. The analysis allowed further comprise an important process underlying, the agency as way to promote therapeutic change in family therapy. We concluded that the open, active and engaged stance of the therapist in the flow of the conversation favored the emergence of these meanings of change.

Definition ceremony: the pathway between the first and second listening in the therapist training process

O artigo apresenta uma experiência de teoria em ação na formação da autora no Instituto Familiae, de São Paulo, entre 2010 e 2014, por meio de um caso atendido no curso em dezesseis sessões. Trata do impacto provocado pela escuta da primeira sessão da terapia familiar, designada como “primeira escuta”, em contraste com as reflexões surgidas, decorridos três anos, o que está sendo nomeado como “segunda escuta”. Desde uma perspectiva sócioconstrucionista e como integrante de uma equipe reflexiva, a autora faz algumas reflexões sobre conceitos como linguagem constitutiva da realidade, empoderamento, posição de não saber e dialogismo, entremeando a discussão desses conceitos, das mudanças que eles propõem e das transformações que promoveram na escuta e no ser/estar em relações da autora com o relato de fragmentos de algumas sessões. O título Cerimônia de Definição é inspirado na abordagem narrativa de Michael White e consolida a apropriação dos recursos adquiridos no percurso da formação e também no processo de escrita deste relato. O artigo foi escrito com base em registros escritos e de áudio das sessões selecionados, mantendo a cronologia original. Os nomes dos participantes foram alterados para preservar o anonimato.

The article presents an experience of theory in action in its author's formation at Instituto Familiae of São Paulo between 2010 and 2014, through a case attended at the Institute in sixteen sessions. It addresses the impact of listening to the first session of family therapy, referred to as "first listening", in contrast to the reflections that have emerged after three years, which is being called "second listening." From a socio-constructive perspective and as a member of a reflecting team, the author makes some reflections about concepts such as constitutive language of reality, empowerment, position of not-knowing and dialogism, amidst the discussion of these concepts, the changes they propose and the transformations they promote listening and in the author's relations with the report of fragments of some sessions. The title Ceremony of Definition is inspired by Michael White's Narrative Approach and consolidates the appropriation of the resources acquired in the formation course as well as in the writing process of this account. The article was written based on written and audio records of the selected sessions, maintaining the original chronology. Participant names have been changed in order to preserve their anonymity.

Reflexive process: an expirience in the public health in between the fight for social inclusion

Este trabalho tem como objetivo compartilhar a utilização da prática da equipe reflexiva, no âmbito da saúde mental, norteada pelos processos reflexivos desenvolvidos por meio das concepções do psiquiatra e terapeuta familiar norueguês Tom Andersen. Apresenta brevemente de que maneira esses aportes teóricos vêm influenciando a prática clínica da autora, relatando a seguir a história de Larissa, paciente que chegou para ser atendida no Centro de Atenção Psicossocial. Larissa denominou de “grande virada” o que ocorreu em sua vida após a conversa ocorrida com a presença de equipe reflexiva, da qual participaram a paciente e as duas equipes de cuidado designadas a ela (hospitalar e CAPS).

This paper's objective is to share the usage of the practice of a reflective team, in the area of mental health, guided by the reflective processes developed through the conceptions of the norwegian psychiatrist and family therapist Tom Andersen. We propose a short introduction on the way those theorical inputs have influenced the author's clinical practice, reporting the following narrative about the patient Larissa who received care at the Psychosocial Care Center. Larissa named "the turning point" what happened in her life after the organized conversation based on the practice of a reflective team, of which the involved were the patient and two care groups designated to her (the hospital's and PSCC's).

The dialogical sex: a facilitator concept to conversations about sexual practices

Frequentemente, em nossas práticas clínicas, nos deparamos com clientes que buscam a terapia com temas relacionados às práticas sexuais, à sexualidade e/ou ao gênero. Desenvolvemos com estes clientes um diálogo sobre os significados que acompanham estes dilemas, explorando e enriquecendo conhecimentos, favorecendo a transformação das formas como nos relacionamos com o fenômeno e a coerência da problematização. Neste artigo, gostaríamos de convidar ao diálogo algumas vozes que participam da significação do encontro sexual, desde tradições antigas e rígidas, a inteligibilidades liberais, de entendimentos culturais, a experiências situadas. Então, propomos uma nova forma de entendimento do encontro sexual, que nomeamos sexo dialógico por suas características intrinsecamente associadas ao diálogo e às transformações que ele gera.

Frequently in our private practices, we meet with clients who seek therapy with themes related to sexual practices, sexuality and genre. Weve developed with these clients dialogues about de meaning that accompany these issues, exploring and enriching knowledge, favoring transformation of ways of relating with the phenomenon and the coherence of problematics. In this article, we would like to invite to dialogue some voices that participate in the meaning making of sexual encounter, since ancient and rigid traditions, to liberal intelligibilities, and cultural understandings to situated experiences. Then we propose a new way of understanding to sexual encounter, which we named dialogical sex, for its intrinsically associated characteristics with dialogue and the transformations that it generates.

Mindfulness in the construction of a communicative space based in attention to the body in therapy

Este artigo discute a incorporação de Mindfulness na terapia conversacional como ferramenta para a construção do espaço terapêutico como espaço comunicativo baseado em atenção conjunta. De maneira específica, aborda o Mindfulness no corpo como ferramenta facilitadora para criar aberturas para novas formas de significação nos fluxos narrativos recorrentes. Os mecanismos de mudança terapêutica associados à prática de Mindfulness (regulação atencional, regulação emocional e descentramento do eu) são discutidos a partir de fragmentos do atendimento de um adolescente com isolamento social, rigidez cognitiva, autoexigência e pensamentos obsessivos. A atenção no corpo a partir de uma perspectiva colaborativa abriu espaço para a mudança terapêutica em um quadro crônico e refratário, com histórico de recaídas e resistente à mudança.

This paper discusses the incorporation of Mindfulness into conversational therapy as a tool for the construction of therapeutic space as a communicative space built on joint attention. Specifically, the paper focuses on Mindfulness on the body as a tool that facilitates the interruption of recurrent narrative flows, creating openings to new forms of constructing meaning. The mechanisms of therapeutic changes associated to Mindfulness (attention regulation, emotional regulation and changes in the perspective of the self) are discussed through fragments from the treatment of an adolescent showing social isolation, cognitive rigidity, self-exigence and obsessive thoughts. The attention to the body from a collaborative perspective created an opening to the therapeutic change in a chronic and refractory case, with a history of relapses and resistance to change.

Women, marriage and career: a systemic feminist perspective

O artigo apresenta uma discussão teórica sobre a vivência de mulheres no casamento e na carreira a partir da perspectiva de gênero e sistêmica feminista. Apresentamos inicialmente uma discussão sobre o feminismo, o trabalho das mulheres e a possibilidade de investir na carreira profissional. Perpassamos aspectos históricos e conceituais que visam à contribuição para a compreensão da experiência das mulheres com a carreira na sociedade contemporânea. Articulamos, em seguida, questões de gênero com reflexões acerca das mulheres no casamento. Por fim, discorremos a respeito da terapia com base na perspectiva sistêmica feminista na interação casamento, carreira e gênero. A discussão ressalta o quanto a perspectiva sistêmica feminista é importante e útil para dar visibilidade às questões de gênero enfrentadas pelas mulheres no contexto do casamento e da carreira.

This paper aims to present a discussion about the experience of women in marriage and in career by using a feminist systemic perspective combined with gender concepts. Initially we propose a discussion about feminism, women`s work and the possibility of having a professional career. We discuss historical and conceptual aspects in order to contribute to the understanding of women`s experience with combining a career and a marriage and family life in contemporary society. Then, it articulate the concepts of gender with the marriage of women. Finally, we describe the feminist systemic therapy in interaction and with a gender and a systemic feminist approach to marriage and family life. The discussion highlights how much the importance of adopting a systemic feminist perspective is important and useful to give light to gender issues dealt by women in their marriage and career.
