Psicanálise e Hospital: uma prática rigorosa

A psicanálise no hospital atua na contramão de qualquer tentativa de driblar o mal-estar na clínica, sustentando a importância do real para todo e qualquer sujeito ali presente. Isso exige uma tomada de posição diante tanto do discurso médico, quanto do científico e até mesmo o dos saberes "psi". Relatam-se alguns dados da própria experiência, que levaram à observação de que, na prática do psicanalista no hospital, não se pode abrir mão da exigência de um rigor não apenas clínico, mas também teórico. Por sua vez, aposta-se que a partir de um trabalho assim construído é possível avançar nesse campo, tanto com pesquisas quanto com o incremento da clínica.

Psychoanalysis in the hospital acts contrary to any attempt to overcome the malaise in the clinic, sustaining the importance of the real for every subject present. This requires taking a stand in the face not only of the medical discourse, but of the scientific discourse and even "psy" discourses, as well. While practicing psychoanalysis in the hospital, some data from our own experience led to the observation that the requirement of rigor can not be waived, neither in the clinic, nor in the theory. On the other hand, it is a possible bet that from such work, we would be able to advance in this field both with research and with the improvement of the clinic itself.

Psicanálise e hospital hoje: o lugar do psicanalista

Este trabalho parte do princípio de que o que define o caráter analítico de uma prática não é o lugar onde ela se realiza, mas é o lugar de onde o operador da Psicanálise faz ato.Nesse sentido, convém dar ênfase ao fato de que quando estamos falando de Psicanálise e Hospital, estamos falando do lugar do psicanalista. Esse lugar não é um lugar dado a priori, e precisa ser construído em cada experiência, de um modo tal que ele, o psicanalista, possa operar. Há três pontos que convém destacarmos para nossa reflexão, neste trabalho: 1) sobre a presença do pensamento psicanalítico no campo da saúde; 2) sobre o potencial transformador da presença do psicanalista no hospital: a entrada e a inserção; e 3) sobre os efeitos da presença do psicanalista na instituição, dando ênfase às consequências e aos problemas decorrentes dessa inserção. Concluímos que, se podemos falar dos efeitos da psicanálise nas instituições de saúde é porque eles evidenciam que o potencial transformador da presença do psicanalista no referido campo não fica sem consequências quando da sua interlocução com os demais campos de saber, pois é o modo pelo qual ele responde (ou não) às demandas a ele dirigidas, sustentando as diferenças discursivas, que possibilita (ou não) a construção do lugar do analista, a realização e os efeitos de seu trabalho na instituição de saúde.

This work assumes that what defines the analytical nature of a practice is not the location in which it takes place, it is where the psychoanalysis takes action. In this sense, it is important to emphasize that when we talk about Psychoanalysis and Hospital, we are talking about the psychoanalyst's place. This place is not given in advance, and needs to be built in each experience, in a way that the psychoanalyst can operate. There are three points that we can highlight for our reflection, in this work: 1) about the presence of the psychoanalytical thought in the health field; 2) about the transforming power of the psychoanalyst's presence in the hospital: the entrance and input; and 3) about the effects of the psychoanalyst's presence in the institution, emphasizing the consequences and the problemas arising from this input.We conclude that if we can speak of the effects of psychoanalysis in health institutions, it is because they show that the transformative potential of the psychoanalyst's presence in the field is not without consequences when he interacts with other fields of knowledge, it responds (or does not) to the demands addressed to it, supporting the discursive differences, which makes it possible (or not) to construct the analyst's place, the realization and effects of his work at the health institution.

A escuta do paciente em cuidados ao fim de vida: entre a ética biomédica e a ética do sujeito

Neste artigo analisamos o papel do profissional de saúde diante dos pacientes sob cuidados paliativos oncológicos. Uma visão protocolar é revisada, associada à ponderação das singularidades identificadas em cada situação. Procura-se analisar questões como o grau de mediação da equipe de saúde nas internações hospitalares e o lugar da escuta das singularidades de cada caso, com suas consequentes incidências sobre os recursos oferecidos pelo saber estabelecido, objetivando a melhoria dos atendimentos realizados. Através da contemplação dos principais sintomas apresentados e da relação das características referentes a esse período, foi possível traçar linhas de conduta introduzidas e mantidas pela equipe de saúde, fornecendo condições para que o sujeito possa tomar um lugar na própria vida, resgatando sua biografia diante o inexorável fim que se aproxima.

The role of the health professional in the context of oncological palliative care is studied, emphasizing the evaluation of the subject and the assistance related to desires, pain and suffering, in their last moments of life. Issues as the degree of mediation of the health team during hospitalization and listening to each case peculiarities, with its consequent impact on the resources usually offered, are analyzed, aiming the improvement of treatment. Through the contemplation of the main symptoms presented, which meaning transcend their regular biological effect and relate exquisitely to this period, it was possible to change the approach of the health team, providing conditions so that the subject can take control of his life, rescuing his biography before the inexorable end that approaches.

Psicanálise no hospital?

O presente trabalho está composto por duas partes. A primeira apresenta uma discussão sobre o exercício da psicanálise na instituição hospitalar, partindo da constatação de que tal prática ainda convoca fortes interrogações sobre sua viabilidade nesse contexto. Especialmente tendo em vista as objeções de operar em um espaço em que a dominância discursiva estaria mais pautada no discurso universitário e do mestre, onde a demanda relativa a um trabalho psicanalítico não estaria propriamente colocada e as condições para o setting analítico estariam inviabilizadas ou pelo menos restringidas. A partir da problematização de proposições apresentadas sobre a impossibilidade da psicanálise no hospital e as consequências dessa premissa, desenvolvem-se as perspectivas de um trabalho de escuta clínica orientado em direção ao impossível. Conclui-se nessa parte do texto que o exercício desse discurso somente é viável desde um referencial ético, fundamentando-se na proposição de uma abertura visando dar lugar à manifestação da singularidade de cada um. A segunda parte apresenta a fundamentação e articulação de duas linhas diretivas: a da medicina, desde a leitura da tela "Medicina" de Klimt, e a psicanalítica, que situa o lugar da medicina a partir da perspectiva lacaniana.

This work is composed by two parts. The first one is a discussion about the psychoanalytic practice at hospitals, considering that this practice still raises questions regarding its viability in this context. Considering that the master's discourse and the university's discourse are predominant in the hospital, it is believed that there is a difficulty in operating the psychoanalytical discourse. Moreover, it is said that there is not a demand towards the psychoanalytical work in a way that the conditions for the analytical setting are unfeasible or at least restricted. Starting with the propositions presented about the impossibility of psychoanalysis at a hospital and the consequences of this premise, the perspectives of a work with clinical listening in the direction of the impossible are developed. In this part of the work it is concluded that the operation of this discourse is only possible from an ethical perspective, which is based on a premise of an openness to make way for each one's singularity. The second part of the work presents the fundamentals and articulations of two lines of work: medicine, read accordingly to the painting of Klimt called "Medicine", and the psychoanalytical, that places medicine in accordance with a Lacanian perspective.

A clínica e a pesquisa psicanalítica no hospital universitário

O exercício da clínica psicanalítica no hospital permite afirmar que se trata de um locus privilegiado para o convite à fala: encontro com o real, surpresa, ameaça narcísica e mal-estar diante da finitude, abrindo um campo vasto para a atuação do psicanalista. Este trabalho discute as especificidades da prática psicanalítica em Hospital Escola e enfatiza a ética da psicanálise de orientação lacaniana na clínica e na pesquisa atualmente. O grande desafio do psicanalista que atua em instituições marcadas pela articulação do discurso da ciência com o discurso capitalista reside em apresentar uma produção que se diferencie radicalmente da produção de objetos de consumo em larga escala. A psicanálise nesse contexto pode produzir pesquisas contundentes por meio do exemplo paradigmático e tratar de sujeitos gravemente afetados pelo gozo mortífero em seus corpos.

The practice of psychoanalytical clinic in the hospital enables us to affirm the hospital as a privileged space for the subject to be invited to speak. Encountering the real, facing surprises and narcissistic threats and feeling discontent in the face of finitude opens up a vast field for the psychoanalyst to act. This article discusses the specificities of psychoanalytical practice in a university hospital, emphasizing the ethics of lacanian psychoanalysis in clinical practice and research in current times. The great challenge faced by psychoanalysts working in institutions marked by the intersection of the science and capitalist discourses lies in being able to produce a form of output that is radically different from the manufacturing of items for mass consumption. The psychoanalysis in this context may engender meaningful research through the paradigmatic example and treat subjects gravely affected by the deadly jouissancein their bodies.

Reflexões sobre a clínica psicanalítica em um ambulatório de especialidades médicas

No presente trabalho são abordadas as particularidades de uma prática orientada pela Psicanálise no ambulatório de um hospital público que realiza procedimentos de alta complexidade. Neste contexto são realizadas as ditas "avaliações psicológicas", que podem representar para o psicanalista um momento precioso de escuta e intervenção. Nota-se ao longo do percurso na instituição como uma intervenção que tem como disparador o encaminhamento médico pode se transformar em uma futura demanda de análise. Há certas especificidades nesta prática, especialmente no que se refere ao viés institucional e suas repercussões na escuta. Trata-se de um contexto em que nos deparamos com múltiplas transferências que demandam diferentes manejos. Com o objetivo de fundamentar teoricamente esta prática, é realizado um breve percurso por alguns dos "Artigos sobre Técnica" de Freud (1911-1915 [1914]/1994) e por textos de psicanalistas contemporâneos. O percurso apresentado traz alguns elementos para uma reflexão a respeito das possibilidades do trabalho analítico no contexto do referido ambulatório.

In the present article, the particularities of a practice guided by psychoanalysis in the ambulatory of a public hospital that performs high complexity procedures are approached. In this context, the so-called "psychological assessments" are performed, representing, to the psychoanalyst, a precious moment of listening and intervention. It is noted along the journey within the institution that an intervention which has, as a trigger, the medical referral can become a future demand for analysis. There are certain specificities in this practice, especially with regards to institutional bias and its repercussions on the listening. This is about a context in which we face multiple transfers that demand different managements. Aiming to theoretically substantiate this practice, a brief course is carried out through some of Freud's "Articles on Technique" (1911-1915 [1914]/1994) and also by texts of contemporary psychoanalysts. The course presented provides some elements for a reflection regarding the possibilities of analytical work in the context of the aforementioned ambulatory.

A psicanálise está em toda parte, e os psicanalistas, em outro lugar

O presente artigo consiste em um relato de experiência sobre a prática psicanalítica em um hospital, em sua relação com a medicina, tendo como eixo a pergunta sobre o que é um psicanalista no hospital. O que permite dizer que há um psicanalista em uma equipe multidisciplinar e o que pode um psicanalista operar com seu ato, o ato analítico? Essas são algumas questões que procuraremos abordar nesse trabalho, visando apresentar o que podem ser os efeitos de uma psicanálise aplicada no hospital, o que significa, em última instância, pensar no ato analítico no campo da medicina, bem como nos discursos que operam nessas diferentes funções.

The present article consists of an experience report about the psychoanalytic practice in a hospital, in its relationship with medicine, based on the question of what a psychoanalyst is in the hospital. Then, we ask: what makes it possible to say that there is a psychoanalyst in a multidisciplinary team and what can a psychoanalyst operate with his act, the analytic act? These are some questions that we seek to address in this work, in order to present what may be the effects of an applied psychoanalysis in the hospital, which means, ultimately, to think about the analytical act in the field of medicine, as well as on the discourses that operate in these different functions.

Prática psicanalítica em enfermaria de pediatria: possibilidades, desafios

Problematizam-se algumas possibilidades e desafios do trabalho do psicanalista numa enfermaria de pediatria de um hospital universitário. Apresentam-se as atividades realizadas nesse contexto, discutindo-se as expectativas da equipe de saúde quanto ao trabalho da equipe de psicologia e possibilidades de intervenção junto à equipe de saúde e ao paciente.

This paper examines some of the possibilities and challenges experienced by a psychoanalyst in a pediatric ward of an university hospital. It presents the activities in this setting and discusses the expectations of the health team concerning the work of the psychology team as well as discussing the possibilities of interventions by the psychoanalyst with the health team and with patients.

Atendimento psicológico durante o pré-natal de risco: ameaça de aborto e hospitalização prolongada

O presente estudo tem o intuito de demonstrar a importância do acompanhamento psicológico durante a hospitalização prolongada devido à gestação de risco. A gestação é um período de intensas transformações físicas e psíquicas para a mulher. Abordaremos um estudo de caso de paciente com incompetência (ou insuficiência) istmocervical (IIC), que permaneceu internada por 90 dias em unidade semi-intensiva de uma maternidade particular. A partir do diagnóstico de gestação de risco, houve a necessidade de a gestante permanecer internada, em repouso absoluto com o intuito de evitar um aborto tardio ou um parto prematuro. Articulando conceitos de Winnicott, com alguns autores contemporâneos, pensaremos o medo de perder o bebê, a angústia, a negação da realidade e a impotência, presentes durante todo o período de internação. É fundamental o acolhimento da gestante pelo serviço de psicologia para que, através de um espaço de fala, possa externar seus sentimentos e elaborá-los.

The present study intends to demonstrate the importance of psychological counseling during long-term hospitalization due to high-risk pregnancy. Pregnancy is a period of intense physical and psychological transformation to women. We will address a case study of a patient with istmocervical incompetence (IIC), who remained hospitalized for 90 days in a semi-intensive unit of a private maternity hospital. Since the diagnosis of high-risk pregnancy, it was necessary for the pregnant woman to remain hospitalized, in absolute rest, in order to avoid a late abortion or a premature birth. In an articulation of Winnicott's concepts with contemporary authors, we will discuss the fear of losing the baby, distress, denial of reality and impotence that were present throughout the whole hospitalization period. The caring for the pregnant woman by the psychology service assistance is fundamental, so that, through a space of therapeutic talk, she can express her feelings and elaborate them.

Contribuições da psicanálise à medicina: grupo com residentes em um programa de cancerologia clínica

Em nossa atuação em uma unidade de internação oncológica nos deparamos frequentemente com o sofrimento dos médicos, que fazem parte do programa de residência em cancerologia clínica de nosso hospital. Dificuldades com o manejo da transferência/contratransferência, precipitadas na relação médico-paciente, a queda dos ideais com relação ao saber, bem como o sentimento de impotência decorrente da impossibilidade curativa do tratamento oncológico geralmente fazem-se presentes nas interconsultas ou, de maneira menos evidente, em produções sintomáticas e acting-outs. Assim, a criação de um grupo, como um espaço de elaboração para tais questões, foi a estratégia de intervenção proposta à coordenação do programa de residência. A partir disso, temos dois objetivos com esse artigo: a) delinear e discutir os temas que se precipitam e se repetem no trabalho em grupo; e b) apresentar o trabalho desenvolvido, discutindo suas estratégias técnicas, metodológicas e teóricas. Utilizamos o diário metapsicológico e a análise de discurso de orientação lacaniana como método de registro e análise das vinhetas clínicas apresentadas. Observamos, através de nosso trabalho, não apenas uma minimização do sofrimento dos residentes médicos, mas também efeitos em sua formação, naquilo que diz respeito ao lugar dado a subjetividade e à transferência na clínica médica.

In our work in an oncology unit we often face the suffering of physicians who belong to the hospital's residence program in clinical cancerology. Difficulties with transference/countertransference management, precipitated in the doctor-patient relationship, the fall of the ideals concerning knowledge, as well as the feeling of impotence due to the curative impossibility of the oncological treatment, usually appears as an issue during the liaison, or, indirectly, through symptomatic productions and acting-outs. Thus, the creation of a group, as a place for elaborating such issues, was the strategy proposed to the coordination of the residency program. Therefore, we have two objectives with this article: a) to delineate and discuss the themes that were often discussed in group work; b) present the work we do, discussing its technical, methodological and theoretical strategies. We used the metapsychological diary and lacanian orientation discourse analysis as a method of recording and analyzing the clinical situations presented. We observe through our work, not only a minimization of the suffering of medical residents, but also effects in their formation, in what concerns the place given to subjectivity and transference in the medical clinic.

Psicanálise aplicada com profissionais em uma instituição hospitalar: os afetos e a dimensão real do Outro

Este artigo constituiu-se com base no relato de experiências de psicanálise aplicada, desenvolvido no escopo de um projeto de pesquisa coordenado pela Profa. Tania Coelho dos Santos e financiado pelo CNPq. Trata-se do desenvolvimento de um dispositivo clínico-institucional para abordar os impasses psíquicos de funcionários que nos permitiram elucidar e intervir em conflitos individuais e coletivos em uma instituição hospitalar. Nos dias de hoje, o hospital enquanto instituição, assim como a instituição familiar, já não contam com as referências tradicionais. Estas se desestabilizaram e assumiram novas formas. Atuar neste ambiente institucional nos exigiu revelar como funcionam a hierarquia, a autoridade simbólica, assim como as fantasias de filiação e pertencimento em tempos pós-modernos marcados pela hegemonia do capitalismo de consumo.

This article was based on the report of some experiences of applied psychoanalysis, developed by Tania Coelho dos Santos as a research project with support of the CNPq budget. It consisted in the development of a clinical-institutional device to address the mental disorders of employees that allowed us to elucidate and intervene in individual and collective conflicts in a hospital environment. Nowadays, the hospital as an institution, as well as the family institution, no longer has the traditional references. These have become destabilized and have taken on new forms. Acting in this institutional environment required us to reveal how the hierarchy, the symbolic authority, and the fantasies of affiliation and belonging in postmodern times marked by the hegemony of consumerist capitalism work.

A escuta psicanalítica no núcleo perinatal: o processo de ser mãe como uma construção

O presente trabalho pretende destacar o lugar da psicanálise em uma enfermaria de gestantes de alto risco no Hospital Universitário Pedro Ernesto (HUPE), apontando o quanto a experiência da maternidade se coloca de forma singular para cada mulher. A partir de fragmentos de casos clínicos e contribuições da psicanálise, articulações como feminino, mito da maternidade e o processo do ser mãe, serão feitas no decorrer do artigo. Seguiremos o estudo afirmando a multiplicidade na experiência do "ser mãe" e basearemos esse ponto com os discursos de pacientes que vivenciam a maternidade em sua singularidade o que aponta para um inesperado na relação mãe-bebê.

The present work intends to highlight the place of psychoanalysis in a high-risk pregnancy ward at Hospital Universitário Pedro Ernesto (HUPE), pointing out how much the experience of motherhood is placed in a unique way for each woman. From fragments of clinical cases and contributions of psychoanalysis, articulations will be made in the course of the article with the subjects of the feminine, the myth of motherhood and the process of becoming a mother. We will follow the study affirming the multiplicity in the experience of "being mother" and we will base this point with the discourses of patients who experience maternity in their singularity which points to an unexpected in the mother-baby relationship.

Hospital e psicanálise: a atuação do psicólogo em um ambulatório de Pediatria

O objetivo deste trabalho é discutir a atuação do psicólogo residente que, orientado pela teoria psicanalítica, exerce sua prática no ambulatório de pediatria de um hospital universitário. Para tanto, faremos uma breve leitura histórica do nascimento do hospital como instituição médica e das transformações propostas pelo SUS nesse tipo de dispositivo. Discutiremos a inserção da psicanálise nesse quadro, bem como traremos fragmentos clínicos de atendimentos com crianças. Interessa-nos abordar as contribuições e a singularidade da psicanálise na "prática entre vários" nos dispositivos do SUS. Neles, a dimensão ética da psicanálise torna-se também política e evidencia-se na sustentação dos vazios de saber como possibilidade de movimento, de novos discursos nas instituições.

This paper aims to discuss the practice of a resident psychologist, whose work is guided by psychoanalytic theory, who practices in the pediatric outpatient clinic of a university hospital. To that end, we will start with a brief historical review of the emergence of the hospital as a medical institution, and the changes proposed by the Brazilian National Health System (SUS) regarding this type of facility. We will discuss the integration of psychoanalysis into this framework, as well as introduce clinical fragments regarding children's care. We are interested in discussing the contributions and singularity of psychoanalysis as it relates to the "practice made up of many different parts" motto at the SUS facilities. At these facilities, the ethical dimension of psychoanalysis also becomes political, as is evidenced by the sustenance of gaps in knowledge as an opportunity for movement, and for new discourses in institutions.

Considerações sobre a prática clínica em uma enfermaria de psiquiatria

A Unidade Docente Assistencial de Psiquiatria do Hospital Universitário Pedro Ernesto, na cidade do Rio de Janeiro, é um dos cenários de práticas do Curso de Especialização em Psicologia Clínica Institucional - Modalidade Residência Hospitalar do Instituto de Psicologia da Universidade do Estado do Rio de Janeiro. Problematizaremos as questões que caracterizam as práticas clínicas institucionais tendo em vista a inserção dos residentes neste dispositivo assistencial, cuja atuação é orientada pela psicanálise.

The psychiatry teaching unit of Pedro Ernesto University Hospital, in Rio de Janeiro, is one of the practices spots of the course in Specialization of clinical and institucional psychology - residency module of the Psychology Institute of the Rio de Janeiro State University. The issues about institucionalized clinical practices will be inquired in this article, whereas the insertion of the residency students on this assistencial set, whose work is guided by psychoanalytical studies.

Psicanálise e hospital universitário hoje

O presente texto aborda questões em torno da inserção da Psicanálise no ambulatório Núcleo de Atenção ao Idoso (NAI), unidade docente assistencial do Hospital Universitário Pedro Ernesto (HUPE), vinculado à Universidade Aberta da Terceira Idade da Universidade do Estado do Rio de Janeiro (UnATI/UERJ), tanto no que tange à relação com a equipe de saúde, quanto à supervisão clínica e institucional, desenvolvida pelas psicólogas que trabalham neste setor, junto às residentes de Psicologia. A inserção em um Hospital Universitário hoje situa, sobretudo, dois significantes: saúde e universidade, fortemente ancorados no progresso da ciência tecnológica. A partir destes significantes, busca-se indicar especificidades recolhidas através da escuta de pacientes reconhecidos pela equipe como "idosos", termo que não expressa as particularidades que aparecem na clínica com estes sujeitos. No entanto, é muito presente na fala destes sujeitos uma incidência de efeitos advindos de perdas importantes e usualmente concomitantes, que podem abrir a necessidade de um trabalho de luto.

The present text addresses questions regarding the insertion of Psychoanalysis in the ambulatory of the Center of Attention to the Elderly (NAI), assistance teaching unity of Hospital Universitário Pedro Hernesto (HUPE), linked to Third Age Open University of the state or Rio de Janeiro (UnATI/UERJ), addressing both the relationship with the health team and the clinical and institutional supervision developed by the psychologists working in this sector, together with the Psychology residents. The insertion in an University Hospital today places, above all, two signifiers: health and university, strongly anchored in the progress of technological science. From these signifiers, it is sought to indicate specificities collected through the listening of patients recognized by the team as "elderly", a term that does not express the particularities that appear in the clinic with these subjects. However, it is very present in the speech of these subjects an incidence of effects arising from important and usually concomitant losses, which may open the need for grief work.

Psicanálise e prática multidisciplinar no hospital: clínica e transmissão

O artigo interroga a questão da transmissão no âmbito da prática do psicanalista em contexto multidisciplinar no hospital. Sem entrar, neste momento, em uma casuística correlativa ao tema, aborda o modo como o problema da transmissão é tratado por Lacan. Recorremos a seu ensino para articular o problema a partir dos eixos que sustentamos aproximar à discussão da experiência que enfocamos: o saber, o semblante e a universalização promovidos pela ciência.

The article questions the transmission within the scope of the practice of the psychoanalyst in a multidisciplinary context in the hospital. Without entering, at this moment, a casuistry correlative to the subject, it approaches the way in which the problem of the transmission is treated by Lacan. We turn to his teaching to articulate the problem from the axis we hold to bring the discussion close to the experience we focus on: knowledge, semblant and universalization promoted by science.

Dispositivos de eficácia: impasses para a Psicanálise no Hospital

Lacan (1966/2001) indica a presença de uma organização industrial, externa ao campo da saúde, como fornecedora de subsídios científicos e tecnológicos, cuja incidência gera a problemática que se constrói com a introdução de medidas de controle quantitativo da eficácia terapêutica. Os modelos de mensuração de eficácia, ao indicarem as constantes estatísticas a serem constatadas em um processo de pesquisa científica, convocam a evidência de sucesso como condição indispensável e essa organização exigirá certa produtividade dos trabalhadores da área. Com o objetivo de questionar a ressonância de tais exigências de eficácia sobre a prática clínica no âmbito hospitalar, sustenta-se que os índices da prática assistencial correspondem ao que se destaca como dispositivo de eficácia. Diante disso, fomenta-se a discussão sobre como o lugar da psicanálise no hospital é atravessado por uma vertente política que implica aspectos clínicos e institucionais de uma prática.

Lacan (1966/2001) indicates the presence of an industrial organization, external to the health field, as a supplier of scientific and technological subsidies, whose incidence generates the problematic that is constructed with the introduction of measures of quantitative control of therapeutic efficacy. Efficacy measurement models, when indicating the constant statistics to be verified in a scientific research process, call for the evidence of success as an indispensable condition and this organization will require a certain productivity of the workers of the area. In order to question the resonance of such efficacy requirements over clinical practice in the hospital setting, it is maintained that the indexes of care practice correspond to what stands out as an effective device. Thus, it is fomented the discussion about how the place of psychoanalysis in the hospital is crossed by a political aspect that implies clinical and institutional aspects of a practice.
