Práxis, Intercessão e Saúde Coletiva: Abílio da Costa-Rosa, ou das verdadeiras paixões pela Psicanálise

Intercessões e análises sobre o Processo de Produção saúde-adoecimento-Atenção no campo psíquico, num Território municipal: produção de novas tecnologias para a implementação da Atenção Psicossocial no Sistema Único de Saúde (2008)

Apresentamos um projeto de pesquisa que visa à produção de novas tecnologias para a implementação da Atenção Psicossocial no Sistema Único de Saúde, por meio de Intercessões e análises sobre o Processo de Produção saúde-adoecimento-Atenção no campo psíquico, num Território municipal. Contextualizados no cenário social e institucional dos campos da Saúde e da Saúde Mental, partindo dos fundamentos teórico-metodológicos e éticos e das definições necessárias, discutimos os processos de produção saúde-adoecimento-Atenção (s-a-A) e Atenção-formação-produção de conhecimento (A-f-pc). Considerando esses dois processos inter-relacionados e problematizando dialeticamente seus diferentes vértices, definimos nossos objetivos gerais e específicos, bem como elencamos os subprojetos integrados nesse projeto matricial. Vinculado, respectivamente, a esses dois processos, consideramos o campo epistemológico e ético que definimos como Intercessão-Pesquisa para, com isso, definirmos os conceitos de Dispositivo Intercessor e de Dispositivo Intercessor como meio de produção de conhecimento, bem como indicamos algumas possibilidades de análises.

We present a research Project that aims the production of new technologies for the implementation of Psychosocial Care in the Unified Health System, through intercession and process of production hea lth - illness - care's analysis in the psychic field, in a municipal Territory. Contextualized in the social and institutional context of the fields of Health ando f Mental Health, based on the theoretical - methodological and ethical foundations and on the nece ssary definitions, we discuss the processes of production health - illness - care (H - I - C) and Care - Training - Knowledge production (C - T - KP). Considering these two processes interrelated and dialectically problematizing their different vertices, we define our gen eral and specific objectives, so we set up the subprojects integrated in this matrix project. Linked, respectively, to these two processes, we consider the epistemological and ethical field that we define as Intercession - Research to define the concepts of Intercessor Device and Intercessor Device as a means of producing Knowledge, so we indicate some possibilities of analysis.

Por que a Atenção Psicossocial exige uma clínica fundada na psicanálise do campo Freud-Lacan?

Discutimos alguns fundamentos de uma clínica fundada na psicanálise do "campo Freud-Lacan" que visa a oferecer Atenção aos impasses psíquicos dos sujeitos, no contexto da Saúde Coletiva, sob a perspectiva do Paradigma Psicossocial definido como superação dialética da Reforma Psiquiátrica. Para tal, se o horizonte ético da Atenção Psicossocial passa a ser a implicação dos sujeitos no desejo e no carecimento é justo propor que isso se traduza num empuxo à Psicanálise. Igualmente, estamos conectados ética e politicamente com o campo da Reforma Sanitária, originária da luta pela implantação da Saúde Coletiva como superação radical da Saúde Pública. Destacamos o imprescindível lugar da "Instituição de Saúde Mental" e dos seus estabelecimentos como intermediários necessários entre os trabalhadores da área e os sujeitos do tratamento. Assim, ao abordarmos a pertinência da psicanálise na Saúde Coletiva, elucidamos três temas fundamentais para avançar nessa direção: 1) adequar o dispositivo institucional a fim de que nele possa caber a psicanálise; 2) a caracterização de um trabalhador de Saúde Mental de um novo tipo, nomeado como trabalhador-intercessor, e 3) as noções de "acontecimento-sujeito" e de "transferência" como primordiais ao trabalho nesse campo. Sublinhamos, ainda, que, no que toca especificamente à psicanálise, nos localizamos no âmbito da ampliação do que Lacan definiu como "psicanálise em intensão", muito longe, portanto, da noção de "psicanálise aplicada".

We discuss some fundamentals of a clinic founded on psychoanalysis of the " field Freud - Lacan " that aims to offer Attention to the subjectivity psychic impasses, in the context of Collective Health, from the perspective of the Psychosocial Paradigm defined as a dialectical overcoming of the Psychiatric Reform. So, if the ethical horizon of Psychosocial Care has became the implication of the subjects in desire and in bedurfnis, it is corrected to propose that this translates into a thrust to Psychoanalysis. Equally, we are connected ethically and politically with the Health Reform field, originated from the struggle for the implementation of Collective Health as a radical overcoming of Public Health. We highlight the indispensable place of the "Mental Health institution" and its establishments as necessary intermediaries between the worker s of the area and the subjects of the treatment. Thus, by addressing the pertinence of psychoanalysis in Collective Health, we elucidated three fundamental themes to advance in this direction: 1) To adapt the institutional device to fit the psychoanalysis; 2) The characterization of a Mental Health worker of a new type, named as worker - intercessor, and 3) The primordial notions of "event - subject" and of "transference" to work in this field. We also emphasize that, specifically on psychoanalysis, we are loca ted in the context of what Lacan defined as "psychoanalysis in Intension", so far from the notion of "applied psychoanalysis".

O Grupo psicoterapêutico no discurso do analista: um novo dispositivo da clínica na Atenção Psicossocial

Procura-se fundamentar teórica e tecnicamente uma nova proposta de grupo psicoterapêutico a partir da psicanálise, com as contribuições de Jacques Lacan: na releitura de Freud, na proposição de uma teoria dos discursos como laços sociais, e em suas considerações sobre a grupalidade. Tomo como ponto de partida e apoio para a reflexão e elaboração um texto importante de Jurandir Freire Costa sobre a teoria e prática dos grupos psicoterapêuticos, procurando fazer uma leitura crítica com base nas contribuições citadas de Lacan, e na minha experiência como psicoterapeuta de grupos e como pesquisador do tema. Partindo da redefinição lacaniana de alguns conceitos importantes na psicanálise de Freud - inconsciente, sujeito, transferência e interpretação - chegamos à proposição de um modo diferente de realizar o grupo psicoterapêutico; de um contorno ainda não praticado para a função do psicoterapeuta; e de uma concepção particular de seu funcionamento e dos seus efeitos. Propomos a redefinição do grupo, assim conceituado, como um possível operador fundamental das práticas da Atenção

We aimed substantiating theoretical and technically a new proposal of psychotherapeutic group based on psychoanalysis, with the contributions of Jacques Lacan: In Freud's rere ading, in a proposition of the discourses theory as social bonds, and in his considerations about the group. I've been taking as a starting point and support for reflection and elaboration an important text of Jurandir Freire Costa about the theory and the practice of psychotherapeutic groups, seeking to make a critical reading based on the contributions of Lacan, and in my experience as a group psychotherapist and as a researcher on the subject. Starting from the Lacanian redefinition of some important con cepts in Freud's psychoanalysis - unconscious, subject, transference and interpretation - we've coming to the proposition of a different way of performing the psychotherapeutic group; of a contour not yet practiced for the function of the psychotherapist; and of a particular conception of its functioning and its effects. We propose the redefinition of the group, conceptuallized as a possible fundamental operator of the practices of Psychosocial Care, in the intercession with the psychic and subjective impas ses that currently present in the field of Collective Health.

Problematizando a instituição pesquisa na universidade e nas práticas psicológicas: questões epistemológicas e éticas

Por meio de uma ampla revisão da literatura procuramos problematizar a pesquisa na universidade e, particularmente, suas relações com as práticas psicológicas, tomando-a como instituição. Para que serve a pesquisa desenvolvida na academia universitária? Quais seriam alguns de seus problemas principais? A partir de uma grade teórica e analítica epistemológica e ética, distinguimos um conjunto de modalidades diversas de pesquisas realizadas na universidade: a pesquisa de corte positivista tradicional; a pesquisa-ação baseada no materialismo histórico; as práticas de intervenção psicossocial de matiz pedagógico/psicologizante; a pesquisa-intervenção referenciada na Filosofia da Diferença e a intercessão-pesquisa orientada por uma complexa perspectiva transdisciplinar, enquanto um modo de produzir transformações sociais, atuando no plano microfísico dos estabelecimentos institucionais nos quais o trabalhador está inserido. As relações entre a pesquisa acadêmica e o plano das práticas psicológicas constituem figuras diversas e também são produtoras de efeitos éticos e sociais distintos. Esse poderia ser um critério para guiar a escolha por uma ou outra dentre tais modalidades.

Through a broad review of the literature we aim to problematize the research in the university and, particularly, its relations with psychological practices, consi deranting they as an institution. What is the purpose of the research developed in the university academy? What are some of its main problems? From a theoretical and analytical epistemological and ethical framework , we distinguish a set of diverse modaliti es of research carried out in the university: the research of tradi tional positivist court; action - research based on historical materialism; the practices of psychosocial intervention based on pedagogical/psychologizing nuance; the research - intervention re ferenced by the Philosophy of Difference and the intercession - research oriented by a complex transdisciplinary perspective, as another way of producing social transformations, acting in the microphysical plane of the institutional establishments where the worker is inserted. The relations between academic research and the plan of psychological practices makes up diverse figures and also produce ethical and social effects different. It could be a criterion to guiding the choice of one or another of these mod alities.

Psicanálise, transdisciplinaridade e Atenção Psicossocial: a necessária formação de trabalhadores de Saúde Mental de um novo tipo

Por meio da análise estrutural da Saúde Mental Coletiva proposta por Costa-Rosa, concebemos que a transposição do Paradigma Psiquiátrico Hospitalocêntrico Medicalizador em direção ao Paradigma da Atenção Psicossocial exige a superação dialética das disciplinas profissionais presentes nesse campo, abrindo caminho para a formação de trabalhadores de um novo tipo. Para tal, tentamos avançar a hipótese de que isso pressupõe a radicalização das contribuições da Psicanálise e do Materialismo Histórico, referências essencialmente transdisciplinares. Diferentemente dos trabalhadores-disciplinares do Paradigma dominante, esses novos trabalhadores deverão interceder junto a impasses psíquicos e institucionais a fim de operar a implicação subjetiva e sociocultural crítica dos sujeitos que a eles demandam ajuda. Concluímos que enquanto os primeiros comungam da ideologia do Modo Capitalista de Produção, reiterando a objetificação alienante dos sujeitos, os trabalhadores-intercessores têm como horizonte a sustentação de uma práxis capaz de pôr os sujeitos no lugar do trabalho da produção, seja da própria cura, no caso dos sujeitos que demandam tratamento, seja de respostas singulares para os impasses socioinstitucionais no trabalho junto aos demais trabalhadores de Saúde Mental.

From the structural analysis of Collective Mental Health proposed by Costa Rosa, we conceive that the transposition of the Psychiatric Hospitalocentric Medicalizing Paradigm towards the Psychosocial Care Paradigm requires the dialectical overcoming of the professional disciplines present in this field, opening the way for the formation o f workers of a new type. So, we try to advance the hypothesis that presupposes the radicalization of the contributions of Psychoanalysis and Historical Materialism, that are essentially transdisciplinar references. Differently from the disciplinary workers of the hegemonic Paradigm, these new workers must intercede in psychic and institutional impasses in order to operate critical subjective and sociocultural implication of the subjects who ask them for help. We conclude that while the first ones share the ideology of the Capitalist Mode of Production, reiterating the alienating objectification of the subjects, the intercessor workers have as their horizon the support of a praxis capable of putting the subjects in the place of the work to produce the cure of themselves.

A pertinência da categoria de ideologia na perspectiva da luta paradigmática em Saúde Mental: traçando novas possibilidades de trabalho

Pretendemos discutir a pertinência da categoria de ideologia para o campo da Atenção Psicossocial e das lutas paradigmáticas em Saúde Mental. Inicialmente postulada por Karl Marx e Friedrich Engels e posteriormente enriquecida por autores ligados à Psicanálise, em especial do campo de Freud e Lacan, esta categoria pode se revelar um instrumento teórico valioso aos partidários da Atenção Psicossocial, permitindo sobretudo mediar uma crítica à ideologia dominante do Paradigma Psiquiátrico Hospitalocêntrico Medicalizador. Assim, buscaremos traçar as linhas gerais desta discussão, em um recorte que atravessará alguns aspectos históricos da Atenção Psicossocial e da Reforma Psiquiátrica brasileira; uma análise dos paradigmas em luta atualmente na Saúde Mental; bem como uma contextualização da categoria de ideologia e uma proposta de sua utilização neste campo. Almejamos, desta forma, fundamentar e abrir novas perspectivas de trabalho, sempre tendo como norte a humanização dos estabelecimentos públicos de Saúde Mental e a concretização dos pressupostos da Atenção Psicossocial.

We intend to discuss the pertinence of the category of ideology in the field of Mental Health and its paradigmatic struggles. Initially postulated by Karl Marx and Friedrich Engels and later enriched by authors related to Psychoanalysis, especially from the field of Freud and Lacan, this category may prove to be a valuable theoretical tool for those who supports the Psychosocial Atention, allowing particularly to mediate a critique o f the dominant ideology of Psychiatric Hospitalocentric and Medicalizer Paradigm. Thus, we will try to outline the general lines of this discussion, pointing some historical aspects of the Brazilian Psychiatric Reform; a analysis of the paradigmatic strugg les in Mental Health; as well as a contextualization of the category of ideology and a proposal of its use in our field. We aim, therefore, to base new perspectives of work, based on humanization of the public facilities of Mental Health and the concretiza tion of the assumptions of Psychosocial Atention.

Do dispositivo disciplinar ao dispositivo intercessor

Este texto tem como mote caracterizar a noção de "dispositivo" a partir de uma leitura que o compreende como tudo aquilo que produz agenciamentos rumo à produção de uma subjetivação singularizante. Para chegar a esse objetivo, utilizamos os exercícios teóricos de Agamben e Costa-Rosa, autores que adotam uma definição singular do conceito de dispositivo, antes cunhado por Foucault e depois trabalhado por Deleuze. Agamben traz a ideia de dispositivo como um conjunto de estratégias circunscritas em relações, jogos de forças, os quais podem produzir redes de captura e um determinando tipo de (des)subjetivação, por isso, tratar-se-á de profanar os dispositivos disciplinares. Costa-Rosa agrega ao conceito de dispositivo o conceito de intercessor - originário de Deleuze, mas relido a partir da psicanálise e do materialismo histórico - formalizando, ou melhor, instrumentalizando, o que, em tese, Agamben propõe ao profanar o dispositivo. Assim, os dois autores parecem formalizar uma aposta na percepção de dispositivo que reconhece como necessária a produção de subjetividades singularizadas.

This text has as a motto to characterize the notion of "device" from a reading that understands it as everything that produces assemblages towards the production of a singularizing subjectivation. In order to reach this goal, we used the theoretical exercises of Agamben and Costa - Rosa, authors who adopt a singular definition of the concept of device, formerly coined by Foucault and later by worked by Del euze. Agamben brings the idea of a device as a set of strategies circumscribed in relations, games of forces, which can produce capture networks and a determining type of (dis) subjectivation, so it will treat to desecrate the disciplinary devices. Costa - R osa adds to the concept of device the concept of intercessor - originating from Deleuze, but reread from psychoanalysis and historical materialism - formalizing, or rather instrumentalizing, what Agamben proposes in defending the device. Thus, the two auth ors seem to formalize a bet on the perception of a device that recognizes as necessary the production of singularized subjectivities.

Dispositivo Intercessor e Atenção Psicossocial: subversão da lógica tecnocrática da Reforma Psiquiátrica contemporânea

Este artigo busca refletir sobre o Dispositivo Intercessor, conceito cunhado por Costa-Rosa, no campo da Saúde Mental, levando em consideração que a Reforma Psiquiátrica e a Atenção Psicossocial são movimentos que se tornaram paralelos no momento atual. Através de uma breve discussão histórico-crítica podemos dizer que há uma Reforma Psiquiátrica inicial, baseada na Reforma Sanitária, e uma contemporânea marcadamente neoliberal. Enquanto inicialmente fazíamos discussões sobre a estrutura político-econômica da sociedade e os atravessamentos na saúde, na Reforma Psiquiátrica contemporânea passamos a fazer uma discussão tecnocrática privilegiando o tema "acesso". As portarias de 2018 para uma nova Saúde Mental e a Nota Técnica 11/2019 do Ministério da Saúde nos interrogam sobre a ênfase na discussão tecnocrática e o "escanteamento" da discussão da Atenção Psicossocial como ética no campo da Saúde Mental. Diante desse cenário retomamos a obra de Costa-Rosa sobre o Dispositivo Intercessor, norte para retomada da Ética necessária para produção de subjetividades singularizadas.

This article aims to reflect on the Intercessor Device, a concept coined by Costa Rosa, in the Field of Mental Health, but taking into consideration that the Psychiatric Reform (PR) and Psychosocial Attention are movements that have become parallel at the moment. Through a brief historical - critical discussion we can say that there is an initial PR based on Sanitary Reform and a contemporary neoliberal one. While it initially made political - economic discussions about the structure of society and its crossing s in health, the contemporary PR started to make a technocratic discussion privileging the theme "access". The 2018 directives for a new Mental Health and Technical Note 11/2019 from the Ministry of Health make us interrogate ourselves about the emphasis o n the technocratic discussion and the scoring of the discussion about Psychosocial Attention as Ethics, in the Field of Mental Health. In view of this scenario, we return to the work of Costa - Rosa about the Intercessor Device, as north to resume the Ethics necessary for the production of singularized subjectivities.

O Paradigma Psicossocial: parâmetros mínimos para as práticas substitutivas ao Paradigma Psiquiátrico Hospitalocêntrico Medicalizador

Servindo-se de uma análise paradigmática das mudanças introduzidas pela Reforma Psiquiátrica brasileira, Costa-Rosa propõe uma análise revolucionária ao conceituar o Paradigma Psicossocial, cujo objetivo é instrumentalizar práticas em Saúde Mental Coletiva substitutivas ao Paradigma Psiquiátrico Hospitalocêntrico Medicalizador. O Paradigma Psicossocial inaugura uma perspectiva sui generis quanto aos modos de se conceber e de se produzir saúde, dando um salto à frente da ideologia liberal, na medida em que pensa a realidade psíquica e a realidade social em continuidade, de modo que o sofrimento psíquico não pode ser devidamente compreendido sem que se discuta o contexto sociocultural que o atravessa e pelo qual é atravessado. Neste trabalho, nosso objetivo é apresentar o Paradigma Psicossocial enquanto um referencial teórico-técnico e ético-político necessário para os avanços do Sistema Único de Saúde no que diz respeito à Atenção Psicossocial.

Costa-Rosa makes a paradigmatic analysis of the changes introduced by the Brazilian Psychiatric Reform going beyond it. He is revolutionary in conceptualizing the Psychosocial Paradigm (PSP), that objective is to instrumentalize practices in Collective Mental Health that are substitutive to the Psychiatric Hospitalocentric Medicalizing Paradigm (PHMP). The PSP has inaugurated a sui generis perspective about the ways of conceiving and produci ng health, taking a leap ahead of the liberal ideology, because it thinks the psychic reality and the social reality in continuity, so the psychic suffering does not can be adequately understood without discussing its socio - cultural context. In this work, our objective is to present the PPS as a theoretical - technical and ethical - political referential necessary for the advances of the Health Unic System in terms of Psychosocial Care.

Medicalização como Sintoma Social Dominante: estratégias a partir do Paradigma Psicossocial

O artigo pretende problematizar a Medicalização como discurso predominante nas práticas de Atenção à na clínica contemporânea. A Psiquiatria DSM, aliada à Indústria Farmacêutica e aos medicamentos psicotrópicos, têm apresentado um modo de olhar para o sofrimento psíquico que tende a eliminar o sujeito, seu contexto histórico e seu saber-fazer sobre aquilo que o adoece. O Discurso Médico transforma os sintomas em transtornos e distúrbios, apostando em pílulas para regular um corpo que fica, assim, reduzido à sua dimensão orgânica. A sociedade contemporânea produz, como um dos efeitos, a medicalização de toda e qualquer manifestação de dificuldades (afetivas e/ou sociais), bem como da dor de existir. Em meio a tanta bioquímica, como a Psicanálise do Campo de Freud e Lacan pode contribuir para que as práticas de "cuidado" se orientem em direção ao Paradigma Psicossocial? A Gestão Autônoma da Medicação (GAM) e o lugar necessário da Psiquiatria e dos psicotrópicos na Atenção Psicossocial parecem ser uma boa aposta.

This article intends to show a predominant medical approach in the practices of Care and Attention to Health in the Contemporary Clinic. DSM Psychiatry, allied to the Pharmaceutical Industry and Psychotropic Medicines, have presented a way of looking at psychic suffering of what a subject, his historical context and his knowledge about what he does. The medical discourse has transformed the symptoms into disorders and disorders, betting on policies for a body that remains, therefore, reduced to its organic vision. "The medicalization of all manifestations of health, health and / or health" as well as the pain of habit. In the midst of biochemistry, such as the Field Psychoanalysis of Freud and Lacan can contribute to the care practices be oriented towards the Psychosocial Paradigm? Autonomous Medication Management (GAM) and Psychosocial appear to be a good bet.

Contribuições do Dispositivo Intercessor para a práxis da Atenção Psicossocial: relato da experiência vivenciada num CAPSad

A construção da Atenção Psicossocial se faz no cotidiano dos Estabelecimentos que compõem a rede de Atenção ao sofrimento psíquico. No entanto, para que estes Estabelecimentos possam empreender práticas congruentes com o Paradigma Psicossocial, é necessário que os trabalhadores sejam capazes de produzir saberes singulares, datados, criativos, pois só eles podem operar como respostas específicas às demandas particulares dos sujeitos do sofrimento que se apresentam às instituições de Saúde. O presente ensaio apresenta o relato de uma experiência de intercessão-pesquisa vivenciada num Centro de Atenção Psicossocial que atende os sujeitos dos impasses decorrentes do uso de Álcool e Outras Drogas (CAPS adII) e as reflexões suscitadas por sua análise. Conclui com indicações importantes de que é possível impulsionar movimentos instituintes a partir do reposicionamento dos trabalhadores em sua relação com a práxis.

The Psychosocial Attention construction is done in the daily life of the Establishments that make up the network of Attention to psychic suffering. However, in order for these establishments to undertake practices consistent with the Psychosocial Paradigm, it is necessary that workers be able to produce singular knowledge , dated and creative, since only they can operate as specific responses to particular demands of the suffering subjects that present themselves. This study presents a case report of an experience of intercession-research lived in a Center of Psychosocial Attention that looks at the subjects of the impasses arising from the Alcohol and Other Drugs using (CAPS ad II) and the reflections raised by their analysis. It´s concluded, with important indications, that it´s possible to boost instituting movements from the repositioning of workers in their relationship with praxis.

Contribuições do Dispositivo Intercessor para o trabalho no campo da Assistência Social

Este ensaio é resultado de processos de intercessões realizadas em estabelecimentos institucionais da política de Assistência Social brasileira. Neste campo marcado pelos significantes "pobreza" e "falta" e pelo discurso sobre o sujeito de direitos, incluímos o sujeito do desejo. Servimo-nos do Dispositivo Intercessor em seus dois momentos de produção: o momento do trabalho junto aos sujeitos da assistência e da prática institucional junto ao coletivo de trabalho; e o momento da reflexão teórica, produzida a posteriori, sobre os processos de produção de intercessões realizadas no primeiro momento. De natureza transdisciplinar, esse dispositivo tem como pilares epistemológicos principais a Psicanálise do campo de Freud e Lacan e o Materialismo Histórico, bem como a Análise Institucional francesa e a Filosofia da Diferença. Visamos trazer algumas contribuições acerca da práxis na Assistência Social, sem a pretensão de produzir um saber totalizante e sim de instrumentalizar outros trabalhadores que desejem se posicionar como intercessores em suas práticas.

This essay is the result of intercession processes that have occurred in institutional establishments of the Brazilian Social Assistance policy. In this field marked by the signifiers "poverty" and "lack" and by the discourse about the law subject, we inc lude the subject of the unconscious desire. We serve of the Intercessor Device in its two moments of production: the moment of work with the subjects of the assistance and of the institutional practice with the collective of work; and the moment of the the oretical reflection, produced a posteriori, about the processes of production of intercessions realized in the first moment. Of transdisciplinary nature, this device has as main epistemological pillars the Psychoanalysis of the field of Freud and Lacan and the Historical Materialism, as well as the French Institutional Analysis and the Philosophy of Difference. We aim to bring some contributions about praxis in Social Assistance, without the pretension to produce a totalizing knowledge, but with the purpose rather to instrumentalize other workers who wish to position themselves as intercessors in their practices.
