In the dialogue between health and the penal system it is possible to note a range of dilemmas that separate the "inside" from the "outside" of prisons and that lead to transformations in the relationships established in these territories (spaces). This article is aimed at understanding how work processes are constructed, along with concepts of subjects in prison, based on the experience of health professionals in the penitentiary health systems. Narrative interviews were held with professionals from Rio Grande do Sul State and the analyses are based on the Schütze perspective. The results suggest that health teams sought to connect with the network of health care, which is situated outside of the prisons. However, they faced obstacles in relation to security operations and established local regulations. They also considered as important that incarcerated subjects are provided with access to health services.
