The present article comes from a clinical case constructed from a care offered to a diabetic patient that presents a long and dramatic trajectory of relation with its treatment. Faced with a history of complications, and after another episode in which he was near death, this space was opened for a work to be carried out under psychoanalytic guidance. In addition to all the questions that could be raised about the performance of the psychoanalyst in the hospital, the cut established for our analysis should focus on the subjective dynamics that can be apprehended in the short time of this treatment. The main focus in discussing this case is therefore a problematization as to the contribution that the Freud and Lacan approaches to diagnosis in obsessional neurosis can offer for the clinical understanding of subjective suffering. The objective is to discuss the subjective condition that presents itself in obsessional neurosis, as well as its strategies against desire and enjoyment, from some key concepts highlighted by Freud and Lacan, such as the subjective relation of confrontation with death, paternal debt, specificities in relation to jouissance and its phallic dimension, as well as the participation of each of these points in the subjective dynamics of the patient.
