This study aims to identify the profile of the users who need home care by a team of the Family Health Strategy. Analytical-descriptive and cross-sectional study with a quantitative approach, developed in a region known as Area 26, in Basic Health Unit União, in Mathias Velho neighborhood, in Canoas/RS. 33 users participated in this study and were included in the home care program. The mean age was 68.9 years, 55% of participants are female and 45% male. As regards comorbidities presented the highest rate (81.81%) was the poor treatment adherence. When related to systemic arterial hypertension (SAH), there was significant score (p = 0.018), indicating that 59.3% of patients with SAH have difficulty adhering to treatment. Using these results, it was possible a multidisciplinary intervention to improve the quality of life of users in full, resulting in greater acceptance and adherence to treatment.
