This paper analyzes the metapsychological hypothesis according to which the subjective impact of a diagnosis of HIV AIDS is traumatic, in the psychoanalytic sense of the term, which means that a clinical setting, which allows clinical unravel the psychodynamics of these patients, conflicts triggered by scientific certainty such as a diagnosis of AIDS, needs to be built. From a clinical vignette the author analyzes the psychological economy resulting - the drive dynamics - that makes the self vulnerable, targeting his, or her, belief of immortality. The author also analyzes its effects on topical mental, ie, the impact for a id, superego, and I. Such a fact brings about the most significant clinical evidence about AIDS: the attack of the superego onto the self, known for all those who engaged in the AIDS clinic, the presence of melancholia, which remains as a research problem.
