This study analyses the meanings, clinical management, and bioethical dilemmas of health professionals in relation to patient (non) adherence to HIV/aids treatment. This is an exploratory, qualitative study, applying content analysis to semi-structured interviews conducted with health professionals from an infectious disease unit in a public hospital. The findings reveal the diversity of meanings attributed to (non) adherence, with the phenomenon predominantly explained from an individualist and rationalist perspective. Faced with (non) adherence, the professionals questioned patient autonomy and addressed the bioethical dilemmas surrounding the secrecy and confidentiality of HIV/aids diagnosis. The findings indicate the need to consider care strategies guided by tailored therapeutic approaches, and to construct arenas for ongoing health education in order to reorient care practices.
