^lpt^aO artigo tem por objetivo discutir o manejo clínico de questões relativas à passagem da infância à adolescência no tratamento do autismo e da psicose em instituição. O trabalho traça as linhas gerais da trajetória de um serviço criado no final da década de 80 com o intuito de atender à clientela mais grave que se dirigia à rede pública de assistência em saúde mental do município do Rio de Janeiro. A partir de alguns fragmentos clínicos, apresenta-se a construção do caso em equipe, a intervenção e seus efeitos. Complementarmente, aborda-se o encerramento do tratamento e o encaminhamento daqueles que finalizam seu percurso na instituição.^len^aThe objective of this article is to discuss the clinical handling of questions relating to the passage from childhood to adolescence in the treatment of autism and psychosis in an institution. The work sketches the general lines in the course of a service created at the end of the eighties whose aim was to assist the most seriously affected clients who came to the public mental health in the municipality of Rio de Janeiro. The article proceeds to examine the direction taken by the clinical-institutional work, which conjoins the practice shared by many with the treatment of the Other carried out by children and adolescents. Starting with some clinical anecdotal examples, we present the construction of the case as a team, the intervention and its effects. Complementing this, we touch on the end of the treatment and the further guidance given to those who finished their stay at the institution.
