This paper describes a preliminary effort to construe a subject's clinical case and propose a pertinent therapeutic plan. The undertaking started the articulation of a wide net of devices and professionals at a small town in the State of Minas Gerais, Brazil. We analyze the obstacles for the exercise of integral health care towards people with mental disorders, as well as the task of making the subject able to contribute to the construction of the clinical case and therapeutic project. This experience has become paradigmatic for mental health care policy, for it has started the use of psychiatric beds at the local General Hospital, and had brought about a new configuration of health care substitute network to psychiatric hospitals.
