This article is a theoretical essay aiming to reflect on the construction of clinical performance in Primary Health Care (PHC) based on Foucault's concept. Through an explanation of a fictitious case, arguments are developed in which we defend that the main biopolitical strategy used in the clinic performed in the PHC currently; is governmentality. We point out some of Foucault's theoretical-methodological tools, inscribed in the being-with-yourself or ethics phase, as possible analysis strategies to think about possibilities for the clinic extension, from the perspective of including the subject in its construction. In addition to the governance of behaviors, which lead to subjection through the use of governance devices, the notions of self-practices, self-knowledge, and parrhesia point to the possibility of building a clinic in PHC guided by an ethical relationship between professionals and health system users.
