According to the definition of the World Health Organization (WHO), palliative care consists in the assistance promoted by a multidisciplinary team that aims to improve the life's quality of the patient and its family, in face of a life threatening disease. In order to understand the psychic functioning of these patients, this study aims to describe aspects of the experience of the Hospital Santa Clara's Psychology Service, part of the hospital network Santa Casa de Misericórdia de Porto Alegre, related to the care provided to terminals patients followed by the team of palliative care, with emphasis on understanding of the Manic Defenses as proposed by Melanie Klein. The use of these defenses by some patients has been identified as a favorable tool in psychological management. This understanding also corroborates the idea that the patient is terminal, but also with therapeutic possibilities.
