This paper aims to promote reflections on the differential diagnosis between symptom and psychosomatic phenomenon (PSF). The question arose from the experience as a psychologist resident in the Integrated Residency Program - Child and Adolescent Health Care at Hospital de Clínicas / UFPR, in the outpatient care of children and adolescents aged 8 to 13 years with atopic dermatitis (AD). The monitoring of children with dermatosis, especially severe cases, raised questions about the treatment direction in an attempt to understand if the disease could constitute a symptom or a psychosomatic phenomenon. To assist in the reflections, the characteristics of the psychoanalytical clinic within a general hospital were presented. Also, the following concepts were recalled: diagnosis in psychoanalysis, symptom in Freud and Lacan, and psychosomatic phenomenon for Lacan. Finally, we conclude on the need to be aware of the status of diagnosis in psychoanalysis, the conceptual categories and the place of the psychoanalyst within the hospital, in order to promote a treatment direction based on the ethics of psychoanalysis.
