The present study is part of a research which studies the functioning of the mental health care network in a city belonging to the Baixada Fluminense region. The objectives are to understand and characterize the demand being met by the Psychosocial Care Center (CAPS) in a 16 months period and to discuss the challenges of implementing qualified assistance directed by the Psychiatric Reform principles. A qualitative method was used with a methodological strategy of document analysis. Results: The team receives an excessive number of new patients, preserves the doctor centered attention model and shows no joint work with the primary attention system. Conclusion: There are important challenges to be faced by the health network, such as the referral of any kind o psychic suffering to CAPS. The Reforma Psiquiátrica's effective implementation demands mental health issues to be rediscussed in various society sectors regarding the production and confrontation of psychic suffering.
