Late life depression is a serious public health issue in Brazil. Cognitive-Behavioral Therapies (CBT) are effective ways of treatment, however there are no available CBT protocol for late life depression treatment in Brazil. This study aimed to conduct the first steps of a cross-cultural adaptation process to Brazil of the American manual "Treating Late-Life Depression - a Cognitive-Behavioral Therapy Approach" that describes an evidence-based CBT protocol to treat late life depression. The following steps were conducted: (1) manual translation from English to Portuguese; (2) reviews of the translated manual made by specialists; (3) incorporation of the feedback from specialists; (4) panel of experts. The main issues discussed were (1) CBT's technical terms translations; (2) adaptation of terms and expressions to the Brazilian culture; and (3) inclusion of updated information. The final version of the manual had semantic, conceptual and cultural equivalence. Studies to investigate the fidelity, viability, acceptability, efficacy and effectiveness of the adapted protocol are still needed.
