This article aimed to analyze the implementation of the Brazilian National Policy of Integral Attention to the LGBTTI+ population, as well as the difficulties of access to health services from the intersectionalities in the transvestite and transsexual experience in the city of Parnaíba, Brazil reality. It is a qualitative study with influence of researches and use of some instruments and tools of ethnographic research, whose sample counted on the participation of health professionals, transvestites and transsexuals. Data were collected through participant observation, systematic production of field diaries and semi-structured interviews and analyzed through discourse analysis. As a result, the policy was not known; the violence in the services, crossed by the intersectionalities of gender and race and social class an aggravating factor of vulnerability. It is concluded that it is necessary to reassess the content of the policy and its implementation so that the above-mentioned issues are thought to be latent for its implementation.
