Rural work in the Brazilian sugarcane sector is marked by hardship, exploitation and imbalanced rights. The objective of this study was to discuss how the restructured work in sugarcane had impacts on the health of sugarcane workers. The methodology involved the analysis of secondary data were obtained from the Social Security website, on the general health conditions, occupational morbidity and mortality of sugarcane workers. It is concluded that the equalization of rights between rural and urban workers covered the amounts of pensions and the restriction of informality, without, however, causing improvements in the general health standard of rural sugarcane workers, as the combination of high-intensity work, long hours and flexible pay contributed to the early deterioration of health.
