His article presents the results of a research that sought to understand how a form of psychosocial care in mental health is constituted from the tensions that are organized around the attempt to consolidate the psychiatric reform in a CAPS in Belém of the Pará. qualitative study of phenomenological orientation made use of participant observation and interviews with ten professionals of the institution as the main source of data. The information obtained was submitted to the phenomenological analysis of the discourse proposed in Ricoeur and Pimentel. The informants' discourses allowed to list at least four tensions existing in the service - political, managerial, interpersonal and operational, that seem to have a direct impact on the modes of care provided to service users. The investment in a service restructuring plan and the institution of regular clinical supervision are suggested as strategies to overcome challenges and tensions.
