Starting from the Lacanian statement anguish is something that never misleads” to the perplexity of the subject before the question “What do you want from me?”, this text makes a historical survey of the first studies on the crises of anguish and the attacks of panic. It recovers the study on the Anxiety Neurosis done by Freud, whose nosographic description was used by Psychiatric Classifications till 1980. In this way, we can see how the term Syndrome of Panic came to replace the previous classification, coming to the name of Panic Disorder. It also discusses the evolution of the concept of anguish and its clinical presentation in individuals subjected to the “real” that invades them. The positions of subject and object come to evidence, revealing the failure of the psychic functioning before a radical helplessness, leading to the fading of the subject. The clinical issues related to psychiatric and psychoanalytic treatment of these patients are brought to discussion through the description of an exuberant symptomatology and the search of the unconscious fantasies that make up the whole picture.
