The necessity of defining a mental representation of one's own self in accordance to the sex to which one belongs constitutes a situation full of dangers. The original psychic bisexuality carries in it difficulties to the perfect completion of the gender identity and as so may impair the synthesis of personality. The tendency of splitting the personality and the correspondent threat to the structuration of the ego are therefore conditions inherent to the essence of the primitive bisexual disposition itself. The identity of sex has its origins in the relations of the individual with the primal scene. To understand the problems created by this fact, one should study the various psychic configurations that, in the multiple game of fusions and separations contained in the primal scene, are produced by the three constitutive parts of it (man, wife e and child). In accordance to some psycho-analytical material of psychotics and sexual perverts, one can consider the presence of three elements separate in themselves and at the same time blended in one conglomerate of three. The opposite processes of identification and differentiation may coexist, due to a peculiar condition at the beginning of the psychic development: the ambiguity. This basic nucleus of the primal scene offers unfoldings and redistributions that are schematically described. The progress in the sense of the reality principle leads to the formation of a gender identity and the sacrificing of the primitive bisexuality contained in the original conception of the primal scene. The relative preservation through sexual perversions of that which the author calls primal hermaphroditic nucleus is a means to preserve the unity of the ego, although the establishment of a perfect identity of the gender might suffer damage. In some cases of masculine homosexuality, one can observe that basically there is an effort to preserve a double identity in order to keep an ego which in other conditions would come to disaggregation. In the author's experience, the masculine transvestism is the form of sexual perversion in which the internal synthesis of both sexes is represented in a more typical way. In schizophrenia the most archaic nature of the clinical picture clearly shows the defensive role of bisexuality. This which has been exposed emphasizes the need of a deeper study of the manner through which the perversion or the restitutive psychotic reaction try through the defense of the original hermaphroditic nucleus to keep or reestablish the integrity of the ego. A better understanding of this subject will be of great value for our theoretical and technical improvement.
