The high rate of suicidal behavior points to the relevance of studying this subject which, in general, is more present in people's lives than it seems to be and has an important impact on social and health services. The Emergency Hospital of the Federal University of Santa Catarina often receives people who attempted suicide and it was identified the need to systematize the visits records made by the department of psychology to these patients. Therefore, this study aimed to develop a recording instrument of psychological care to attempted suicide patients attended by the Emergency. The researcher sought to develop a recording form to fit the reality of the services provided in the context of emergency. As a result, it was made a succinct instrument that contain the main points that should be evaluated in this situation and important information for future follow up and interventions.
