A reflection is proposed on the role of trauma in the structuring of the psychic apparatus. We develop the Freudian theory that addresses current neuroses, seeking to show that current neuroses are the somatic representation of the drive, marks without psychic representation that are expressed by bodily discharges, accompanied by feelings of fear, which perpetuate the traumatic effect. The hypothesis is that the mother's unconscious has a traumatic effect on the baby, promoting, at the same time, primary repression and the foundation of the Ego and, in the case of failure of the maternal function, the formation of areas of representational void, as an effect of commotion and anticipation. The clinical management of these patients has some special features, with the effectiveness depending on the analysis of empathy and identification, to enable, per via de porre, the construction of new psychic areas. The classical interpretative approach is ineffective and may have a re-traumatizing effect.
