The objective of this article is to reflect about the concepts of true and false self and their manifestations in contemporary clinic; as well as the possibilities of intervention using Winnicott’s psychoanalysis. The false self concept proposed by Winnicott is of great value to think about the recurrents reports of persons that search for analysis after experiencing intense anguish episodes and emptying of the sense of life, and simultaneously they cannot find, neither in their affective relationships nor in their professional lives, anything that could justify such feeling of unreality and emptiness. Through the discussion of a clinic case, the concepts of true and false self are articulated with the practice and open the reflection to the analyst’s mirror role in the management of those cases.
