This study aimed to understand the health perception of remaining population of quilombolas in the city of Canoas / RS. A qualitative approach has been applied, with a sample of 12 adults living in the Chácara das Rosas Community, who were submitted to a semi-structured interview, followed by content analysis according to Minayo. Three central categories were revealed: Health-disease process and self-care from the perspective of quilombola´s reality; Individual and socioenvironmental factors determining health disease processes; Inefficient social and health care systems associated with reduced empowerment to exercise health rights. It was found that there was a distorted perception of health, disease and self-care, as well as social determinants of health related to the urban environment, social disinterest and inefficiency of the health sector for the ethnic group care. The ignorance of quilombolas in relation to the National Policy of Integral Health of the Black Population with reduced participation in spaces of social control has also been detected.
