ABSTRACT Introduction: Positive end-expiratory pressure-zero end-expiratory pressure (PEEP-ZEEP) is a physical therapy technique for bronchial clearance. Objective: To evaluate the acute effects of PEEP-ZEEP compared to manual hyperinflation (MH) in critically ill tracheostomized patients in relation to safety and efficiency for bronchial hygiene. Methods: Individuals submitted to invasive mechanical ventilation through a tracheostomy tube were randomized into two groups, one that received the PEEP-ZEEP technique first, and another that received MH. The collection of cardiorespiratory variables was performed before, 1 minute and 30 minutes after each technique, as well as the weight of the aspirated pulmonary secretion. Results: For 18 patients with a mean age of 66.4 (17.3) years, no differences were found between groups for any of the hemodynamic and respiratory variables. The amount of pulmonary secretion removed was also similar between the techniques. Conclusion: PEEP-ZEEP and manual hyperinflation do not differ in relation to the clearance of bronchial secretions and cardiorespiratory variables.
