Upon considering the psychoanalytic treatment of patients affected by sudden facial disfigurement as well as patients with cognitive sequelae due to stroke or traumatic brain injury (TBI), it was possible to investigate different forms of psychic (re) organization. Both the experience of facial shattering and the manifestation of cognitive sequelae lead to primary defensive modes. In such cases, the therapeutic function of the traumatic allows the updating of primitive traumas, providing an opening for an elaboration work and, consequently, the emergence of new questions to be investigated.
