Abstract

Through the experience in public ambulatory care the phenomenon of a new chronicity of the substitution devices in mental health has become evident. Users have become dependent on care, reporting, over time, the aggravation of symptoms. Under analysis, they have appeared to be dominated by a strong sense of emptiness, producing demands that transcended the setting - states in which the inconsistency of psychic limits brings a difficulty in the expression of thought and symbolization. The strong bond with the institution, configured as a transference addiction, has proved to be a strategy for these individuals to deal with their narcissistic fragilities.
