The psychiatric reform is a growing process which does not occur in a consensual way throughout the world. It is related to the peculiarities of economic, historical, political and cultural conditions that characterize the different regions and countries. As a social practice, it is formed by a collective way, including professionals, relatives, users and the society as a whole. The present work aims to analyse the significance given by the relatives of mental patients to the families of mental patients and the social inclusion in the present context of the psychiatric reform. The work took place in João Pessoa, Paraíba, and involved 60 relatives of mental patients who were attended in a psychiatric hospital and in a unit of the Psychosocial Attention Center (CAPS). The data were collected by the Free Association of Words Test which was analyzed through the Tri-Deux-Mots software. The results showed that the mental patient`s families considers they have attributions such as prejudice, overload, unhappiness and suffering, which constitute a great obstacle to the social and familiar inclusion of the mental patient, hindering the acceptance of the psychiatric revolution and its tenets.
