This article works with data of researches that points to the existence of heterogeneity in the constitution of the field of work of the therapeutic accompaniment and mental health institutions. Such heterogeneity derives from the influence of psychoanalytic discourses and social insertion of mentally affected patients. Its counter-transference effects are discussed from the ethical point of view, since they lead to the establishment of a relationship in which the professional detects insufficiencies in his patient and tries to attend them or feels he is unable to deal with them or discovers the existence of a kind of rivalry between him and patients relatives. We conclude considering the importance of such aspects to be dealt with in the clinical supervision.
