The mother-baby relationship is fundamental to the child's psychological constitution. When this relationship does not protect the baby's incipient psychism, he may use his somatic system to express a psychic overload, producing psychofunctional symptoms. This study aimed to understand the mother-baby relationship in the presence and absence of baby's psychofunctional symptom (PS), through a multiple case study design. Four adult mothers and their babies took part in the study. Clinical and sociodemographic data were collected and M.I.N.I. Plus, Symptom Check-List, Interview on Pregnancy, Childbirth and Motherhood Experience and Interaction Assessment Procedure were apllied to. The results showed that mothers whose babies did not present PS had no greater difficulties in dealing with the demands and challenges of motherhood. On the other hand, when the infants presented PS, the mothers presented signs of apathy and depression, experienced stressful situations and showed less capacity to solve them. The study allowed a deeper understanding of the mother-baby relationship and its association with the baby's psychological health, indicating the importance of early intervention on these manifestations
