Introduction: Contamination of HIV among young people is increasing and adherence to treatment became a challenge for public health. Objective: Evaluate the association between adherence and personality characteristics in young HIV-positive patients in two hospitals. Hypothesis: Adherence is influenced by personality characteristics. Methods: The sample consisted of 63 patients, 16 women (25,4%) and 47 men (74,6%), age between 18 and 32 years (average 25,6, standard deviation 3,67) that came to the services during one year. The ASSIST test was used to evaluate abuse of chemical substances, the "Questionnaire to Evaluate Adherence to HIV Treatment" to evaluate adherence and the Factorial Battery of Personality to access personality characteristics. Results: Conscientiousness, Effort, Vulnerability and the 109 and 112 items of the FBP were independently associated with adherence. Those patients who keep their viral load detectable have less chance to adherence in the future. Conclusion: Including these predictors in the general evaluation of patients can improve adherence. A psychologist as part of the team is important because it is possible to develop more adaptive personality characteristics through counseling.
