This present study proposed to analyze and evaluate defensive resources of doctor-clowns working in hospitals and reactions to death and mourning of those institutionalized. Twenty volunteer clowns, that work in hospitals in the metropolitan region of São Paulo, collaborated with the research. Data was collected through a questionnaire for characterization of the sample and application of the Desiderative Questionnaire (QD), between May and June 2016. Desiderative Questionnaire proved to be a smart tool to access the defensive resources, as well as dynamics, functions, impulses and egoic sanctions of the subjects, considering the nature of this tool that places the subject to deny human life and identify with another being, and thus, how he deals with the death drive. The content present, both in discourse and in QD cathexis, point out that clown doctors have to face, in the voluntary work, challenges that go beyond laughing and playing, and that the egoic resources used to deal with anxiety and anguish appear more resistant in much of the sample. It was considered the important that the volunteers participate in therapeutic accompaniment so that the work can continue in an environment that deals with characteristics that are not very permissive to recreational activity.
