In the present article, the particularities of a practice guided by psychoanalysis in the ambulatory of a public hospital that performs high complexity procedures are approached. In this context, the so-called "psychological assessments" are performed, representing, to the psychoanalyst, a precious moment of listening and intervention. It is noted along the journey within the institution that an intervention which has, as a trigger, the medical referral can become a future demand for analysis. There are certain specificities in this practice, especially with regards to institutional bias and its repercussions on the listening. This is about a context in which we face multiple transfers that demand different managements. Aiming to theoretically substantiate this practice, a brief course is carried out through some of Freud's "Articles on Technique" (1911-1915 [1914]/1994) and also by texts of contemporary psychoanalysts. The course presented provides some elements for a reflection regarding the possibilities of analytical work in the context of the aforementioned ambulatory.
