This article presents the basis of the psychoanalytic approach of Lacan´s orientation on transsexualism and its prerogative of changing sex. The diagnostic hypothesis of psychosis sustained by Lacan is analyzed as opposed to the diagnosis of gender identity disorder supported by Stoller, and by the contemporary psychiatric classification of mental disorders. The effects of transsexual enjoyment on the certainty about identity, as well as on the very strong desire for sexual reassignment and the passage to the act of surgery, are analyzed. The way each approach understands sex differences and their consequences is discussed: according to the biological approach, sexual difference is an effect of the organism's biological determinants, and transsexualism, a typical phenomenon of the disjunction between sex and gender; on the other hand, the analytical perspective considers sexual difference an effect of discourse, and transsexual enjoyment, a bent outside the sexual order. Based on Lacan´s orientation, the effects of transsexual enjoyment on the certainty about identity, as well as on the very strong desire for sexual reassignment and the passage to the act of surgery, are elucidated.
