Many changes in the excluding practices over psychiatry’s background have created the contemporary modes in treatment of psychosis. With the establishment of the significant user, the person with serious mental disorder is regarded as a subject with rights whose disease is not a definitive exclusion sentence, but the possibility of transience. This work presents the development of an assistance with a young psychotic who uses a public health service, through which it is possible to notice the new relations with the great Other. From the delusional stories brought to the sessions by the young man, it was possible to create a minimum gap between the user and the primordial Other. This space has allowed the apparently fragmented identity in psychosis to be constructed minimally, though permeated by its radically delusional form. Finally, the paper discusses the importance of establishing a clinic that does not ignore the socio-historical context of capitalist production, thus, as counterpoint against the cataloging required by DSM-IV.
