Home care is a health assistance modality that aims to prevent diseases or to promote the early discharge of patients with chronic diseases and the optimization of inpatient unit. These patients are, usually, elderly, suffering by many diseases. The present text focalizes the psychologist work by the experience at the Núcleo de Assistência Domiciliar Interdisciplinar (NADI) of the Instituto Central do Hospital das Clínicas da Faculdade de Medicina da USP. Discussions are held around peculiarities of the work in a multiprofessional team and the psychologist activities in the domiciliary setting, with their limitations and possibilities. The author presents possible forms of psychological assessments and interventions including the patient, the caregiver, the family and the health team.
