Childhood cancer brings several challenges, changing not only the life of the sick child as well as the family system. The aim of this study was to carry out a systematic review of the literature about children that have a sibling with cancer and their experiences since the diagnosis. The search of published articles between 2012 and 2017 in MedlineLilacsScopusScience Direct, Pubmed and Scielo databases revealed 17 relevant studies of different designs, however, none Brazilian study on the subject was found. The analysis of those publications allowed the identification of three major axes addressed by the authors: school performance, family functioning and the sibling’s feelings. The data indicated that the siblings suffer significant consequences in relation to the disease, perceived mostly as negative, although they identify potentials of growth. Thus, it is evident the need to expand the interventions and researches directed to this population, in order to effectively meet their psychosocial needs.
