ABSTRACT After some discussions at Raul Soares Clinic Session, it was possible to reflect over the impasses of the psychosis treatment in a mental Health institution, which has as an objective a "Psychiatry Reform". If the If the ideal of social insertion and the autonomy preconceived by the reform were considered universalized, disregarding any kind of particularity, they may be, many times, inductors of a breakdown and not a reaction of stabilization in psychotics patients. Considering the case Márcia, can be demonstrated as interventions based in the institutional ideal and not in the construction of a subjectivy to permit an instability, as the agitation psychomotor and hallucinations. By this means, it is necessary to conduce the treatment precisely, disregarding the social insertion; autonomy; but, considering a social string in psychosis.
