ABSTRACT For four years, is has being executed at Raul Soares, the IRS Clinic Session: There is a place designed to discuss the clinical cases. This place of weekly discussions, is an strategy of intervention in the institution an in the Construction of the Clinic Case. The case, which are considered the most difficult and which requires from the institution an answer, attend the Clinic session, which has as approach an interview with the patients as well as a presentation of the case by the group responsible for the patient. Inspired by the work of Lacan, the clinical session brings the mark of the institution psychoanalysis articulation: Psychoanalysis, applied bringing a singular dimension of each case, trying to make it favorable to construct each case and making it possible for the group to act accordingly to the practical logic done by many. In this article will be worked, from this experience, the notion of the discussion in counter- balance to the construction of the clinical case
