The proposition of a public health policy for users of alcohol and other drugs occurred in Brazil, in 2003, influenced by the experience of Psychiatric Reform and the STD / AIDS program from the Harm Reduction strategies. The objective of this work is to discuss the care provided to the user of alcohol and other drugs from the perspective of harm reduction in a small city in Minas Gerais. As a methodological resource was used the description of a case, comparing it to the relevant literature. Before the case, the proposal of the mental health team is to position yourself and act within the human values and the principles of psychiatric reform and harm reduction, in order to enable listening spaces to trace the possible path to follow from the wishes and choices of the subjects involved. In conclusion, attention to user of alcohol and other drugs should be given from a network of care, and harm reduction as a strategy and guiding paradigm for understanding the phenomenon of drug use in societies.
