This research discusses the psychoanalytical methodology of investigation - construction of the clinical case - as a resource in the treatment of subjective urgencies. The word brought by the subject who arrives at the psychiatric hospital is collected as the guiding point of the treatment, while the narratives of the family and institution are also summoned to compose the case. From the construction of João's clinical case, we verified the rupture that occurs in the statement of the psychotic subject in front of the other as an absolute and concrete instance, causing the urgency to erupt. The position of secretariat adopted by professionals in the figure of the analyst, however, raises fragments of discourse that gradually give support to the subject's body-house. This unprecedented solution that he presented offers to the practice elements to think about possible treatments to the Other and the passage to the act in psychosis.
