The text tells the trajectory of the construction of a mental health service in which we can observe the exclusion as a point in common between the mental health service itself and its clients; besides, there is a process of identification and recognition of wishes and sufferings connecting the professionals and the patients. In this History, the public power, health professionals and the patients as the subjects of the action; and this action as the determinant of the change.
