This review is an attempt to broaden the knowledge about Gestalt therapy, bringing the history of this psychology approach and the basic concepts used by it, as awareness, figure-ground, here and now, dialogical relationship, Kurt Lewin field theory, Kurt Goldstein organismic theory and Gestalt Psychology. To assimilate how is the clinical practice through these concepts was necessary to review and add about the theoretical understanding of the self theory (id functions, ego and personality) and adjustments resulting from it (neurotic, distressing and psychotic), being depth in this search the knowledge of psychotic or creation adjustments as a function impairment of the Id, which is understood as the fund experienced of the system self, which is spontaneous and there is no control over it and can make many experiments seem meaningless in given time. These adjustments are divided into background fill adjustments, background articulation and defense against demand or isolation adjustments (autism). The present study focuses on seeking review and add knowledge about the adjustments of defense against the demand or isolation (autistic adjustments); share the practice that was bibliographically investigated, and also share the experience of the psychologist and the implications that this type of practice for these professionals, knowing that this is a restricted area in scientific production within the same approach and requires more theoretical-experiential, including to improve the practice of Gestalt therapists who have affinity or experience in this area
