The cognitive-behavioral therapy (CBT) is now considered an effective and evidencebased adjunct treatment for schizophrenia. The current cognitive model for schizophrenia has been developed individually for their main symptoms: hallucinations, delusions and negative symptoms. The delusions of persecution are the most frequently type found among patients with schizophrenia. Although the efficacy data, it is still little widespread the use of CBT for psychotic patients. This work aims to do a reading based on the cognitive model of a previously reported case of paranoia by Freud. The case chosen was selected from a search of the complete works of Freud, observing the following inclusion criteria: the patient should have paranoid delusions, have been personally treated by Freud and in his account submit personal data sufficient for making the cognitive conceptualization. This revision study identifies characteristics of the cognitive model Beck, for example, the importance of cognitive distortions in maintaining paranoia, in one case described in the early twentieth century. Moreover, one can conclude that paranoid patients pose challenges to therapists of various historical epochs.
