The psychiatric reform, in placing the issue of hegemony of the medical-psychiatric knowledge in treating severe psychosis and neurosis suffering in the public assistance, has opened space to the building of knew forms of psychic suffering though the relating of different knowledge and practices. This work approaches the stalemate and challenges of the clinical practice developed in the field of mental health, in particular to the new services created from the Brazilian Psychiatric Reform, as the “Centers of Psychosocial Attention” (CAPs). We have tried to analyze the place of the clinic along with the institutional actions, taking into account the plurality of clinical devices which are present in the psychosocial attention daily-life where we highlight the medical psychiatric device of attention brought by the reform and the psychoanalytical device.
