This study aimed to investigate the alcohol consumption among residents of agrarian settlements in Rio Grande do Norte and Piauí, identifying usage patterns and the relationship with the living conditions of the population. Moreover, it aimed to problematize the professional action and qualification in these areas. We developed a descriptive exploratory study in 15 rural settlements in both states (RN = 9 and PI = 6). We applied a sociodemographic questionnaire and the AUDIT in 1,999 people (752 men and 1,247 women). The families were composed by up to 5 people, mean age of 47 years, earning between ½ and 2 times the minimum wage, predominantly primary school educational level, source of income was family agriculture, services, pension, and government benefits. The study showed that the living conditions, weaknesses in the infrastructure of the settlements, gender, income, access to health and educational services are factors related to problematic use of alcohol. These results indicate the need for guiding professional practices and qualification concerning determinants of mental health in rural populations.
