This study aims to describe organizational variables of three Psychosocial Alcohol and Drug Care Centers (Caps AD) in terms of their activities, coverage, available resources, client and employee profiles, practices, and therapeutic strategies, as well as how much care is based on the governmental guidelines of the psychosocial model. This descriptive exploratory study analyzes norms, governmental guidelines, and archival data for two of the Caps AD located in the Brazilian Federal District and one in São Paulo. The data were submitted to qualitative (categorical tables) and quantitative analysis (descriptive statistics). The results indicate problems with the infrastructure, territorial coverage different from the recommended one, difficulties clients have accessing services, communication between the staff and the network of psychosocial care centers, adversely affecting service provision, patients' adherence to treatment, and staff performance, which indicates fragile alignment between the services offered and governmental guidelines. The effective application of this model depends upon a solution to these problems.
