This paper pretends to evaluate the actions of harm reduction developed in some brazilian penitentiaries, with the support of the Ministry of Health. For this aim, prisoners, penitentiary agents, authorities and health professionals of each institution were interviewed. The actions of harm reduction, that emerge of the proposal of diminishing the harmful consequences from the consumption of drugs, without necessarily require the abstinence, facing particular resistances in this type of institutions. In particular, the penitentiary doctors and agents were the most resistant to support and involve in these actions. The prisoners participate and support these initiatives, thus allowing to obtain some significant results. The consumption of crack, which comes gradually substituting the use of cocaine in its injectable kind, demand more original and particular forms of intervention that must be discussed with the interested population.
